摘要
目的:分析双多普勒超声评估胎儿心律失常的应用价值。方法:选取200例产前检查疑似胎儿心律失常的孕妇作为研究对象,对孕妇实施双多普勒超声检查,以临床确诊结果为参照,计算双多普勒超声对胎儿心律失常的诊断灵敏度、特异度、准确率,计算其对不同类型胎儿心律失常的诊断符合率。结果:双多普勒超声对胎儿心律失常的诊断灵敏度、特异度、准确率分别为96.85%(123/127)、95.89%(70/73)、96.50%(193/200);以产后心电图检查结果为准,双多普勒超声对房性期前收缩、室性期前收缩、窦性心动过缓、房室传导阻滞、窦性心动过速等类型胎儿心律失常的诊断符合率分别为93.88%(46/49)、92.59%(25/27)、95.00%(19/20)、94.12%(16/17)、92.86%(13/14)。经一致性分析,双多普勒超声诊断胎儿心律失常的结果与临床确诊结果之间的一致性良好,Kappa值为0.729。结论:双多普勒超声可对胎儿心律失常予以灵敏准确检出,可作为胎儿心律失常筛查的主要手段。
Objective:To study application value of duplex Doppler ultrasound in evaluating fetal arrhythmia.Methods:200 pregnant women suspected of having a fetus with arrhythmia during prenatal examination were selected as the research subjects,and given duplex Doppler ultrasonography.By using the clinical diagnosis results as reference,the diagnostic sensitivity,specificity,and accuracy of duplex Doppler ultrasound in the diagnosis of fetal arrhythmia were calculated.Further,the diagnostic coincidence rates for different types of fetal arrhythmias were also calculated.Results:The diagnostic sensitivity,specificity,and accuracy of duplex Doppler ultrasound for fetal arrhythmia were 96.85%(123/127),95.89%(70/73)and 96.50%(193/200),separately.Using the postpartum electrocardiogram results as reference,the coincidence rates of duplex Doppler ultrasound in the evaluation of atrial premature contraction,ventricular premature contraction,sinus bradycardia,atrioventricular block and sinus tachycardia were 93.88%(46/49),92.59%(25/27),95.00%(19/20),94.12%(16/17)and 92.86%(13/14),separately.The consistency analysis showed that there was a good agreement between the results of duplex Doppler ultrasound for fetal arrhythmia and clinically confirmed results with a Kappa value of 0.729.Conclusions:Duplex Doppler ultrasound can detect fetal arrhythmia sensitively and accurately.Therefore,it can be used as the main means of screening fetal arrhythmia.
