无精症患者血清性激素水平及遗传学分析 被引量:1

Analysis on Levels of Serum Sex Hormones and Genetics in Azoospermia Patients

摘要 目的:探究无精症患者血清性激素水平变化,并分析患者遗传学特征。方法:将2017年1月-2019年4月本院收治的无精症、少精症患者及同期体检健康者作为研究对象,分别纳入无精症组(n=310)、少精症组(n=91)及健康对照组(n=110)。随后均进行血清性激素水平测定,同时进行Y染色体AZF微缺失检测与染色体核型分析。比较三组血清性激素水平差异,Y染色体AZF微缺失情况,染色体核型异常情况,Y染色体AZF完整与缺失、染色体核型异常与正常无精症患者性激素水平。结果:无精症组FSH、LH水平均显著高于少精症组与健康对照组,T水平显著低于少精症组与健康对照组(P<0.05);无精症组与少精症组Y染色体AZF微缺失发生率(6.45%和4.40%)均明显高于健康对照组(P<0.05);无精症组染色体核型异常发生率(19.03%)显著高于少精症组(4.40%)和健康对照组(0.91%)(χ^2=11.382、21.783,P<0.05);Y染色体AZF完整与缺失无精症患者性激素如FSH、LH及T水平比较,差异均无统计学意义(P>0.05);染色体核型正常无精症患者性激素如FSH、LH水平均明显低于染色体核型异常无精症患者(P<0.05),T水平明显高于染色体核型异常无精症患者(P<0.05)。结论:无精症患者血清性激素水平异常,这种异常状态与染色体核型关系密切,而与Y染色体AZF是否完整无关。对少弱精患者联合检测血清性激素、Y染色体AZF微缺失和染色体核型具有重要意义。 Objective:To explore the changes in levels of serum sex hormones in azoospermia patients,and to analyze their genetics features.Method:The azoospermia and oligozoospermia patients who admitted to our hospital from January 2017 to April 2019,and healthy people who underwent physical examination in the same period were enrolled as study objects.They were included into azoospermia group(n=310),oligozoospermia group(n=91)and healthy control group(n=110).Subsequently,levels of serum sex hormones were measured,AZF microdeletion detection of Y chromosome and karyotype analysis were performed,the differences in levels of serum sex hormones,AZF microdeletion detection of Y chromosome and abnormal chromosome karyotype among three groups,and levels of serum sex hormones among azoospermia patients with Y-chromosome AZF integrity and absence,normal and abnormal chromosome karyotype were compared.Result:The levels of FSH and LH in azoospermia group were significantly higher than those in oligozoospermia group and healthy control group,while T level was significantly lower than those in oligozoospermia group and healthy control group(P<0.05).The incidence of Y chromosome AZF microdeletion in azoospermia group and oligozoospermia group(6.45%,4.40%)were significantly higher than that in healthy control group(P<0.05).The incidence of karyotype abnormalities in azoospermia group(19.03%)was significantly higher than those in oligozoospermia group(4.40%)and healthy control group(0.91%)(χ^2=11.382,21.783,P<0.05).There were no significant differences in levels of sex hormones such as FSH,LH and T in azoospermia patients with Y-chromosome AZF integrity and absence(P>0.05).The levels of FSH and LH in azoospermia patients with normal karyotype were significantly lower than those with abnormal karyotype(P<0.05),while T level was significantly higher than that with abnormal karyotype(P<0.05).Conclusion:The levels of serum sex hormones are abnormal in azoospermia patients,this abnormal status is closely related to chromosome karyotype,while not related with integrity and absence of Y chromosome AZF.It is of great significance for the combined detection of serum sex hormone,AZF microdeletion of Y chromosome and karyotype in patients with oligospermia and weak sperm.
