摘要
目的:探究在指导困难动静脉内瘘建立中血管超声造影与彩色多普勒超声的应用价值比较。方法:选择接受治疗并存在困难动静脉内瘘的终末期肾脏病患者100例,随机分为两组,其中观察组采用血管超声造影指导建立,对照组采用彩色多普勒超声指导建立,再对比两组手术基本指标、术后内瘘成熟率以及并发症发生率。结果:观察组手术时间为(114.5±13.8)min,术中出血量为(19.5±4.8)ml,均显著低于对照组,一次性成功率为96.0%,术前定位与手术部位匹配率为98.0%,均显著高于对照组,差异具有统计学意义(P<0.05)。观察组手术1周后闭塞率为90.0%,1个月后成熟率为94.0%,2个月后可穿刺率为100%均显著高于对照组,差异具有统计学意义(P<0.05)。结论:采用血管超声造影指导困难动静脉内瘘建立,促进术后内瘘成熟,效果确切,值得临床推广采用。
Objective To explore and compare the application value of vascular contrast echocardiography and color Doppler ultrasound for guiding difficult arteriovenous fistula establishment.Method 100 cases of patients with end-stage renal disease with difficult arteriovenous fistula and who received treatment were selected,and were randomly divided into two groups.The observation group were guided by vascular contrast echocardiography,while the control group were guided by color Doppler ultrasound.Then,the basic indexes of operation,the rate of internal fistula maturation after the operation and the incidence of complications of two groups were compared.Results The operation time of observation group was(114.5±13.8)min,the amount of bleeding during operation was(19.5±4.8)ml,The one-time success rate was 96.0%.The matching rate of preoperative localization and surgical site was 98.0%,which was significantly higher than that of the control group(P<0.05).The occlusion rate was 90.0%after one week and 94.0%after one month.The puncture rate was 100%after two months in the observation group,which was significantly higher than that in the control group(P<0.05).Conclusion The vascular contrast echocardiography in guiding difficult arteriovenous fistula establishment,and promote the maturity of postoperative internal fistula,and the clinical effect is accurate,thus it is worthy to be applied in the clinical.
