摘要
目的 观察精细解剖保留肋间臂神经(ICBN)的改良式乳腺癌根治术的临床疗效及对术后功能的影响.方法 选取2015年6月~2018年6月于我院实施改良式乳腺癌根治术的80例乳腺癌患者为研究对象,随机分组,其中40例观察组患者精细解剖完整保留ICBN,40例对照组患者不保留ICBN,比较两组手术效果、并发症及对术后功能的影响.?结果?两组的手术时间、术中出血量、术中清扫淋巴结数目比较差异无统计学意义(P>0.05);术后1、3、6个月,观察组的患侧上臂感觉功能障碍发生率均显著低于对照组(P<0.05);两组的术后并发症(皮瓣坏死、伤口感染及淋巴漏)发生率比较差异无统计学意义(P>0.05);出院后,两组患者经门诊或电话随访1年,无远处转移和死亡,两组的复发率比较差异无统计学意义(P>0.05).结论 在改良式乳腺癌根治术中,精细解剖保留ICBN不影响手术质量,不增加并发症发生率及术后复发率,且能明显减轻患者上臂感觉障碍,值得推广应用.
Objective To observe the clinical efficacy of modified radical mastectomy with intercostobrachial nerve(ICBN)reservation by fine anatomy and its effects on postoperative function.Methods 80 patients with breast cancer who underwent modified radical mastectomy at our hospital from June 2015 to June 2018 were selected as the study subjects,and randomly divided into two groups,among which ICBN was kept intact in fine anatomy in the observation group of 40 patients,while ICBN was not retained in the control group of 40 patients.The operative effect,complications and their effects on postoperative function were compared between the two groups.Results There was no statistical significance in operation time,intraoperative bleeding volume,and number of lymph nodes dissected during operation between the two groups(P>0.05);at 1,3 and 6 months after the operation,the incidence of sensory dysfunction of patients’upper arms in the observation group was significantly lower than that in the control group(P<0.05);there was no statistical significance in the incidence of postoperative complications(flap necrosis,wound infection and lymphorrhagia)between the two groups(P>0.05);after discharge,there was no distant metastasis or death in the two groups of patients after 1 year of outpatient or telephone follow-up,and the recurrence rate of the two groups was not statistically significant(P>0.05).Conclusion In modified radical mastectomy,ICBN reservation by fine anatomy does not affect the operative quality,does not increase the incidence of complications and postoperative recurrence rate,and can significantly alleviate sensory dysfunction of patients’upper arms,which is worthy of promotion and application.
