摘要
目的观察加味泽泻汤联合敏使朗治疗痰湿型梅尼埃病的临床疗效。方法将168例符合梅尼埃病(痰湿型)诊断标准患者按照随机数字表法分为治疗组和对照组,分别给予敏使朗治疗及在此基础上加用加味泽泻汤治疗,2组治疗前后均采用眩晕障碍量表(DHI)、耳鸣致残量表(THI)评价眩晕及耳鸣的变化情况。2组中各有54例样本在用药前后采用了中医证候评分表评价中医证候变化情况。结果治疗组临床治疗有效率显著高于对照组(P <0.05);治疗组DHI、THI评分及中医证候评分均较前下降,治疗组较对照组下降更为显著(P <0.05),治疗组中短病程组(病程<30个月)较长病程组(病程≥30个月)疗效更优(P <0.05)。治疗组在改善恶心呕吐、耳鸣及视物旋转方面优于对照组。结论加味泽泻汤联合敏使朗在治疗痰湿型梅尼埃病时可以减轻内淋巴积水,能够改善梅尼埃病患者的临床症状,且短病程较长病程疗效更优,值得临床推广。
Objective To observe the clinical effect of modified Zexie decoction combined with merislon in the treatment of phlegm-dampness Meniere’s disease. Methods 168 patients with Meniere’s disease(phlegm dampness type) were divided into treatment group and control group according to the method of random number table. They were treated with merislon alone and added with modified Zexie decoction. Before and after treatment, the changes of vertigo and tinnitus were evaluated by DHI and THI. In addition, 54 samples of each group were evaluated by TCM syndrome score table before and after the medication, which evaluated the changes of TCM syndromes. Results The effectiveness in the treatment group was significantly higher than that in the control group(P<0.05);The scores of DHI, THI and TCM syndrome score in the treatment group were lower than before, and the scores in the treatment group were more significantly lower than those in the control group(P<0.05). In the treatment group, the therapeutic effect in the short course group(course<30 months) was better than the long course group(course≥30 months)(P<0.05). The treatment group was better than the control group in improving nausea and vomiting, tinnitus and visual rotation symptoms. Conclusion In the treatment of phlegm-dampness Meniere’s disease, modified Zexie decoction combined with Merislon can relieve endolymphatic hydrops, which can effectively improving clinical symptoms of patients with Meniere’s disease. And the short course group is better than long course group, which is worthy of clinical promotion.
