摘要
目的:探讨胸部肿瘤放射治疗时,患者双手摆放位置对其摆位误差的影响。方法:将2017年2月至2019年2月收治的83例胸部肿瘤患者纳入本次的研究中,所有患者均采取三维适形放射治疗,根据放疗时患者双手的摆放位置分为对照组( n=41)与观察组( n=42),其中对照组患者在放疗时双手置于身体两侧,而观察组患者则双手抱肘置于额头上,对两组患者的摆位误差进行统计与对比。 结果:在本次研究的测量结果中,观察组患者在左右方向的摆放误差为(2.32±1.21)mm,上下方向的摆放误差为(2.54±0.98)mm,前后方向为(2.10±0.51)mm,而对照组患者的对应误差分别为(3.68±1.43)mm、(3.55±1.01)mm、(2.11±0.46)mm,两组患者在左右(LR)及上下(SI)方向的摆放误差差异有统计学意义,其中对照组患者的摆放误差比观察组患者更为明显( P<0.05),但两组患者在前后(AP)方向的摆位误差却较为一致,并无统计学差异( P>0.05)。观察组患者在左右、上下以及前后方向的校准频率分别为23.0%、21.2%以及26.4%,而对照组患者的校准频率分别为31.3%、28.0%、33.7%,观察组患者在左右(LR)、上下(SI)及前后(AP)方向的校准频率明显低于对照组患者( P<0.05)。 结论:在胸部肿瘤患者的放射治疗中,采取双手上举的方式进行体位固定,可有效减少平行方向的摆位误差,降低其放疗校准频率,值得应用与推广。
Objective To explore the effect of the position of the hands on the setup error of patients with thoracic tumor receiving radiotherapy.Methods Eighty-three patients with thoracic tumor treated in our hospital from February 2017 to February 2019 were included in this study.All patients were treated with threedimensional conformal radiation therapy.The patients were divided into control group(n=41)and observation group(n=42)according to the position of their hands during radiotherapy.The patients in the control group were placed with their hands on both sides of the body during radiotherapy,while the patients in the observation group were held with their elbows on their foreheads.The setup errors of the two groups were compared.Results In the measurement results of this study,the setup error of patients in the observation group in the left-right direction was(2.32±1.21)mm,the setup error in the superior inferior direction was(2.54±0.98)mm,and the setup error in the anterior-posterior direction was(2.10±0.51)mm,and those in the control group were(3.68±1.43)mm,(3.55±1.01)mm,and(2.11±0.46)mm respectively.It could be seen that the setup errors of patients in the control group in the left-right and superior inferior direction were more significant than those in the observation group,with statistically significant differences between the two groups(P<0.05);but the setup errors in the anterior-posterior direction of the patients of the two groups were more consistent,without statistically significant difference(P>0.05).The calibration frequency of the observation group was 23.0%,21.2%,and 26.4%in the left-right,superior inferior,and anterior-posterior direction,those in the control group were 31.3%,28.0%,and 33.7%,the calibration frequency of the observation group in the left-right,superior inferior,and anterior-posterior direction were lower than those in the control group(P<0.05).Conclusion In the radiotherapy for patients with thoracic tumor,the position of upper extremities up can effectively reduce the setup error in the parallel direction,and reduce the calibration frequency of radiotherapy,which is worthy of application and promotion.
