摘要
目的分析消炎痛灌肠与布洛芬灌肠对早产儿动脉导管未闭(patients ductus arteriosus,PDA)心功能、微炎状态的影响。方法择取我院2016年6月~2019年6月收治的80例PDA早产儿,依随机数字表法分为对照组和治疗组各40例,对照组患者采用消炎痛灌肠治疗,治疗组患者采用布洛芬灌肠治疗,治疗7 d后,对比两组患者的心功能、微炎状态、不良反应发生情况。结果两组治疗7 d后组间LVESD、LVEDD、LVEF、PGE2、BNP、ET-1指标比较,差异无统计学意义(P>0.05)。组内对比LVESD、LVEDD、PGE2、BNP、ET-1均显著低于治疗前,差异有统计学意义(P<0.05),组内LVEF比较,差异无统计学意义(P>0.05)。治疗组不良反应发生率(5.00%)显著低于对照组(25.00%),差异有统计学意义(P<0.05)。结论消炎痛灌肠与布洛芬灌肠均可有效改善PDA早产儿心功能及微炎状态,但布洛芬灌肠避免了首过效应,不良反应更少,值得借鉴。
Objective To analyze the effect of indomethacin enema and ibuprofen enema on cardiac function and micro inflammation of preterm infants with patent ductus arteriosus(PDA).Methods 80 preterm infants with PDA admitted in our hospital from June 2016 to June 2019 were divided into control group and treatment group using the random number table method.The patients in the control group were treated with indomethacin enema,and the patients in the treatment group were treated with ibuprofen enema.After 7 days of treatment,the cardiac function,micro inflammatory state and adverse reactions of the two groups were compared.Results There was no significant difference in left ventricular end-systolic dimension(LVESD),left ventricular end-diastolic dimension(LVEDD),left ventricular eject fraction(LVEF),prostaglandin E2(PGE2),brain natriuretic peptide(BNP)and endothelin-1(ET-1)after 7 days of treatment between the two groups(P>0.05).The levels of LVESD,LVEDD,PGE2,BNP,ET-1 after treatment were significantly lower than those before treatment,and the differences were statistically significant(P<0.05),but there was no statistical difference in LVEF(P>0.05).The incidence of adverse reactions in the treatment group(5.00%)was significantly lower than that in the control group(25.00%),and the difference was statistically significant(P<0.05).Conclusion Both indomethacin enema and ibuprofen enema can effectively improve the cardiac function and micro inflammation of preterm infants with PDA,but ibuprofen enema can avoid the first-pass effect,with fewer adverse reactions,which is worthy of reference.
