摘要
目的探讨急性磷化铝吸入性中毒的临床特点及治疗策略。方法回顾分析2019年7月福建省立医院急诊科收治的磷化铝中毒合并急性白细胞减少病例6例的临床进程、转归并探讨其临床特征。结果6例均为轻症磷化铝中毒患者,主要临床表现为恶心、呕吐、胸闷、头晕。所有患者均出现急性白细胞减少,并在24h内未经治疗即恢复正常。大部分患者出现一过性胆红素升高与乳酸升高等临床反应,经高流量吸氧、糖皮质激素抗炎以及对症支持治疗后均恢复正常。所有患者随访2个月,上述指标未见明显异常。结论磷化铝中毒在发病早期可能出现白细胞减少,应密切观察并及时处理。密切随访与积极的支持治疗对改善预后起到关键的作用。
Objective To investigate the clinical characteristics and treatment strategies of acute aluminum phosphide inhalation poisoning.Methods Six cases of inhalation poisoning with acute aluminum phosphide complicated with leukopenia in the emergency department of our hospital in July 2019 were reviewed,and the clinical data were collected and analyzed.Results All the 6 patients presented with mild aluminum phosphide poisoning.The main clinical manifestations were nausea,vomiting,chest tightness and dizziness.Acute leukopenia occurred in all patients and the count of WBC returned to normal within 24 hours without specific treatment.Most of the patients had transient elevated bilirubin and elevated lactic acid,and both of the indices returned to normal after high flow oxygen therapy,glucocorticoid therapy and symptomatic support treatment.All patients were followed up for 2 months and there was no significant abnormality in the above indices.Conclusion Aluminum phosphide poisoning may induce leukopenia in the early onset of the disease and it should be closely observed and dealt with in time.A close follow-up and active supportive treatment play a key role in improving the prognosis.
