摘要
目的:对比Dyract复合材料、光固化复合树脂与玻璃离子水门汀对楔状缺损牙体修复效果及牙周组织的影响,为临床楔状缺损牙修复提供指导。方法:根据填充物的不同,将2014年1月-2017年1月笔者医院口腔门诊诊治的166例(467颗患牙)楔状缺损牙患者分为三组。A组:59例(165颗患牙),采用Dyract复合材料填充;B组:53例(142颗患牙),采用光固化复合树脂填充;C组:54例(160颗患牙),采用玻璃离子水门汀填充。所有患者随访1年,观察临床疗效以及对牙周组织的影响。结果:166例患者均进入结果分析,与A组相比,C组在填充物固位、边缘密合、色泽协调、边缘着色、表面形态满意度方面均较低(P<0.05),B组色泽协调、边缘着色满意度较低(P<0.05);B组继发龋齿、敏感牙、牙髓病发生率较高(P<0.05),C组牙体缺损发生率较高(P<0.05),B组和C组修复成功率均较低(P<0.05);C组牙周龈沟液量、碱性磷酸酶(ALP)、天冬氨酸转氨酶(AST)水平较高(P<0.05),B组菌斑指数、龈沟出血指数较高(P<0.05);B组和C组牙周组织30°、90°夹角应力较大(P<0.05)。结论:Dyract复合材料对楔状缺损牙体修复效果最好,对牙周组织影响最小。
Objective The effects of Dyract composite,photocurable composite resin and glass ionomer cement on the restoration of wedge-shaped defect and periodontal tissue were compared,in order to provide guidance for the clinical treatment of s wedge-shaped defect.Methods According to different fi ller,166 patients(467 diseased teeth)with wedgeshaped defect were divided into three groups from January 2014 to January 2017 in the department of stomatology of our hospital,group A(59 cases,165 diseased teeth)were fi lled with Dyract composite,group B(53 case,142 diseased teeth)with photocurable composite resin,group C(54 cases,160 diseased teeth)with glass ionomer cement.All patients were followed up for 1 year to observe the clinical effi cacy and the effect on periodontal tissue.Results The results of 166 patients were analyzed.Compared with group A,group C had lower satisfaction with fi ller retention,edge density,color coordination,edge coloring,and surface morphology(P<0.05),while group B had lower satisfaction with color coordination and edge coloring(P<0.05).The incidence of secondary caries,sensitive teeth and pulp diseases was higher in group B(P<0.05),and the incidence of dental defects was higher in group C(P<0.05),the rate of repair success was lower in both group B and group C(P<0.05).Gum furrow fl uid volume,and the levels of alkaline phosphatase(ALP),aspartate aminotransferase(AST)were higher in group C(P<0.05),and the plaque index and gingival crevicular hemorrhage index were higher in group B(P<0.05).The stress between the periodontal tissues of group B and group C was greater at 30°and 90°(P<0.05).Conclusion Dyract composite has the best effect on the restoration of wedge-shaped defect and has the least effect on periodontal tissue.
