富血小板纤维蛋白联合GBR技术在种植体周围炎骨缺损重建的临床应用 被引量:26

摘要 目的探讨富血小板纤维蛋白(platelet rich fibrin,PRF)联合引导性骨再生技术(guided bone regeneration,GBR)在种植体周围炎骨缺损重建治疗中应用的临床效果。方法选取就诊于哈励逊国际和平医院口腔医学科行种植修复合并种植体周围炎的患者共计90例,对照组30例行翻瓣刮治,观察组60例,其中30例接受Bio-Oss骨粉植入后覆盖生物膜治疗,另外30例接受PRF和Bio-Oss骨粉混合物联合植入后覆盖PRF生物膜治疗。术后定期复诊,观察并统计种植体周围黏膜生长、骨再生、骨重建成功率等临床指标。结果Bio-Oss+生物膜+PRF组在术后种植体周围黏膜生长情况、骨再生方面均优于Bio-Oss+生物膜组和翻瓣治疗组(P<0.05);3组在骨重建成功率方面无差异(P>0.05)。结论PRF联合GBR技术应用于种植体周围炎骨缺损重建治疗,其在促进软组织愈合、促进骨重建等方面效果显著,值得临床推广使用。 Objective To investigate the clinical effect of platelet-rich fibrin(PRF)combined with guided bone regeneration(GBR)in the reconstruction of peri-implant inflammatory bone defects.Methods A total of 90 cases of patients admitted to Hudson international peace hospital for implant-repair combined with peri-implant inflammation were selected.30 patients in the control group received flap therapy.60 patients in the experimental group.Among them,30 of the patients were treated with Bio-oss bone meal implantation and then covered with biofilm,and another 30 patients were treated with PRF and Bio-oss bone powder mixture after implantation and covered with PRF biofilm.Postoperative follow-up was conducted regularly,and clinical indicators such as growth of mucosa around the implant,bone regeneration and success rate of bone reconstruction were observed and counted.Result Postoperative mucosal growth and bone regeneration in the Bio-oss+Biofilm+PRF Group was better than the Bio-Oss+biofilm Group and the flap treatment Group(P<0.05).There was no difference in the success rate of bone reconstruction among the 3 groups(P>0.05).Conclusion The application of PRF combined with GBR in the reconstruction of peri-implant inflammatory bone defects has significant effects in promoting soft tissue healing and bone reconstruction,which is worthy of clinical application.
