摘要
目的探讨血清镁与急性脑卒中后睡眠障碍的相关性。方法收集神经内科收治的急性脑卒中发病7天内入院的患者318例,记录性别、年龄、文化程度、婚姻状况、体重指数、家庭收入、吸烟、饮酒、高血压、糖尿病、高脂血症、卒中史、卒中类型、前后循环、血清镁等情况,同时评定美国国立卫生研究院卒中量表(NIHSS)评分。1月后随访,进行匹兹堡睡眠质量指数(PSQI)、汉密顿抑郁量表24项(HAMD)、改良的Rankin评分(MRS)、Barthel指数(BI)、简易智能精神状态检查表(MMSE)评分。同时在我院体检中心招募100名健康对照者,记录血清镁及一般情况,评定PSQI、HAMD评分。分析血清镁与PSQI的关系。结果急性脑卒中患者的血清镁低于健康对照组,差异有统计学意义(P<0.05)。脑卒中后一月,睡眠质量差组与睡眠良好组的血清镁、NIHSS评分、HAMD评分、MRS评分、BI差异均有统计学意义(P<0.05)。二元Logistic回归分析显示,HAMD评分、血清镁与急性脑卒中后睡眠障碍独立相关(P<0.05)。结论急性脑卒中患者血清镁浓度较健康对照者低,且在脑卒中后影响睡眠质量。
Objective To investigate the correlation between serum magnesium and sleep disorders after acute stroke.Methods 318 patients admitted to the Department of Neurology within seven days after the onset of acute stroke were collected.Sex,age,education level,marital status,body mass index,family income,smoking,drinking,hypertension,diabetes,hyperlipidemia,stroke history,stroke type,anterior and posterior circulation,serum magnesium,etc.were recorded.Meanwhile,NIHSS score was assessed.After one month of follow-up,Pittsburgh sleep quality index(PSQI),Hamilton depression scale(HAMD),modified Rankin scale(MRS),Barthel index(BI)and mini-mental state examination(MMSE)were conducted.Meanwhile,100 healthy controls were recruited from the physical examination center,serum magnesium,general data,PSQI and HAMD were evaluated.The correlation between serum magnesium and PSQI was analyzed.Results The level of serum magnesium in patients with acute stroke was lower than that in healthy controls,and the difference was statistically significant(P<0.05).One month after stroke,there were significant differences in serum magnesium,NIHSS,HAMD,MRS and BI between poor sleep group and good sleep group(P<0.05).Binary logistic regression analysis showed that HAMD score and serum magnesium were independently correlated with sleep disorders after acute stroke(P<0.05).Conclusions Serum magnesium level of patients with acute stroke is lower than that in healthy controls,and it affects sleep quality after acute stroke.
