摘要
目的:探析电针治疗原发性骨质疏松症(primary osteoporosis,POP)临床研究的选穴规律。方法:检索PubMed、CNKI数据库,纳入公开发表的关于电针治疗POP的临床研究文献。运用文献计量学方法和Excel 2010对文献中的选穴进行总结分析。结果:共纳入文献19篇,使用腧穴49个,总使用频次165次。治疗方法统计依次为:单用电针(26.32%)、电针结合中药(21.05%)、电针结合西药(10.53%)、电针结合理疗(10.53%)、电针结合穴位注射(5.26%)、综合疗法(26.32%);明确指出电针连接穴位的有8篇(42.11%),未提及的有11篇(57.89%);明确电针正负极连接穴位的有2篇(10.53%);明确电针所选波形的有16篇(84.21%),其中疏密波9篇(37.50%),连续波7篇(43.75%),未提及波形的有3篇(18.75%);使用频次居前10的腧穴为肾俞、脾俞、命门、足三里、委中、关元、阳陵泉、悬钟、夹脊穴、阿是穴;最常用的特定穴为背俞穴、五输穴及交会穴;穴位在经脉的分布依次为膀胱经、督脉、胆经、任脉、胃经、脾经、肾经、大肠经、肝经、心包经。结论:电针治疗骨质疏松症(osteoporosis,OP)选穴以膀胱经、督脉及胆经穴位为主,注重特定穴,强调病证结合、远近配穴,凸显治病求本、标本同治的原则。
Objective:To discuss the acupoint selection rules of electroacupuncture for treating primary osteopo-rosis(POP)through analyzing relevant clinical research.Methods:The PubMed and CNKI databases were searchedfor published clinical research on treating POP with electroacupuncture.The bibliometric method and Excel 2010 were used to summarize and analyze the selection of acupoints in the literature.Results:A total of 19 studies wereincluded,involving 49 acupoints with a total frequency of 165 times.The treatment methods included electroacu-puncture(26.32%),electroacupuncture combined with traditional Chinese medicine(21.05%),electroacupuncturecombined with Western medicine(10.53%),electroacupuncture combined with physical therapy(10.53%),elec-troacupuncture combined with acupoint injection(5.26%),and comprehensive therapy(26.32%).Eight studies(42.11%)documented the acupoints electric needles were connected to,while the remaining 11(57.89%)did notmention them;2 studies(10.53%)indicated the acupoints positive or negative electrodes were connected to.Wave-forms were declared in 16 studies(84.21%),where 9 studies(37.50%)used a disperse-dense wave and 7 studies(43.75%)used a continuous wave,while the remaining 3 studies(18.75%)did not mention waveforms.The top 10 acupoints in use frequency were Shenshu,Pishu,Mingmen,Zusanli,Weizhong,Guanyuan,Yanglingquan,Xuan-zhong,Jiaji,and Ashi points.The most commonly used specific acupoints were back-shu,five-shu,and crossingpoints.The meridians that contained the acupoints were as following in descending order of the number of acupoints:bladder meridian,governor vessel,gallbladder meridian,conception vessel,stomach meridian,spleen meridian,kidney meridian,large intestine meridian,liver meridian,and pericardium meridian.Conclusion:Electroacupunc-ture for POP mainly involves the acupoints on the bladder meridian,governor vessel,and gallbladder meridian,with an emphasis on the specific acupoints.The therapy underlines an integrated consideration of disease and syn-drome and a combination of far and near acupoints,following the principle of aiming at root causes and also alleviat-ing symptoms.
