摘要
肝硬化门脉高压症食管-胃静脉曲张破裂出血(esophageal-gastro variceal bleeding,EGVB)的预防和治疗一直是肝硬化并发症防治的重点,国内外诸多相关指南和专家共识均给出了指导意见。但随着多中心、大样本量临床试验的不断开展和循证医学证据的渐趋丰富,目前的EGVB防治策略中已出现不少可商榷或待明确之处。本文综合最新的国内外相关指南、专家共识和临床试验数据,就其中主要问题进行讨论,同时提供系统性的EGVB防治策略图,为临床实践提供参考,为临床研究提供新思路。
The prevention and treatment of esophageal-gastro variceal bleeding(EGVB)in patients with cirrhotic portal hypertension is a critical issue in the management of the complications of cirrhosis.Tremendous guidelines and expert consensus have provided recommendations in clinical practice.However,with the continuous development of multi-center,largesample clinical trials and evidence-based medicine and the richer evidences available,some of the recommendation in the current strategies for the prevention and treatment of EGVB may require further discussion.The latest domestic and foreign relevant guidelines,expert consensus and clinical trials data are summarized and the main issues are discussed.Meanwhile,a systematic prevention and treatment strategy map of EGVB is introduced to provide the reference and new ideas for clinical practice and research.
