摘要
目的观察微型锁定钢板治疗髌骨冠状面骨折的临床效果。方法选取北部战区总医院自2016年12月至2018年12月收治的26例髌骨冠状面骨折患者为研究对象,所有患者均接受微型锁定钢板治疗,观察治疗效果。结果术中关节面均复位满意,无关节软骨缺损,固定可靠。所有患者切口均Ⅰ期愈合。所有患者术后均获随访,随访时间6~15个月。所有患者骨折均顺利愈合,未出现骨折再移位或螺钉、钢板松动断裂等。末次随访时,患者膝关节活动度达105°~135°,平均110°,疗效满意,无切口感染、排斥反应、关节强直、创伤性关节炎、内固定物激惹等并发症。根据Bostman标准评定:优21例,良5例,优良率100.0%(26/26)。结论微型锁定钢板符合髌骨的生物力学特点,提供了更高的生物力学稳定性,软组织激惹少,能够有效固定含有关节软骨的冠状面骨折块,临床治疗中能够对骨折部位进行牢靠的固定,患者可早期进行膝关节屈伸功能锻炼,最大程度恢复膝关节功能,还具有骨折恢复快、并发症少、优良率高等优点。
Objective To observe the clinical effect of miniature locking plate in treatment of patella coronal fracture.Methods A retrospective study was performed on 26 cases of patients with patella coronal fracture who were admitted from December 2016 to December 2018.All patients were treated with miniature locking plate and the therapeutic effect was observed.Results During the operation,the reduction of articular surface was satisfactory,no defect of articular cartilage was found,and the fixation was reliable.All the incisions were healed in stage I.All patients were followed up after the operation for 6 to 15 months.All the patients′fractures healed successfully,and there was no fracture redisplacement or loosening of screws or plates.At the last follow-up,the range of motion of the knee joint reached 105°to 135°,with an average of 110°,and the curative effect was satisfactory.According to Bostman standard evaluation,excellent in 21 cases,good in 5 cases,excellent and good rate was 100.0%(26/26).Conclusion Miniature locking plate in accordance with the biomechanical characteristics of the patella,provides greater biomechanical stability,less soft tissue irritability,can effectively fixed which contain coronal fractures of articular cartilage,clinical treatment can firmly down on fracture in patients with early functional exercise of knee joint flexion and extension,restore the knee function to the greatest extent,rapid recovery,fewer complications,and also has a higher excellent and good rate.
