丹红注射液对急性心力衰竭合并快速心律失常患者血清神经元特异性烯醇化酶 脂连蛋白 氨基末端脑钠肽前体及炎症因子的影响 被引量:14

Effect of Danhong injection on serum NSE,APN,NT-proBNP and inflammatory factor in patients with acute heart failure complicated with rapid arrhythmia

摘要 目的探讨丹红注射液对急性心力衰竭合并快速心律失常患者血清神经元特异性烯醇化酶(NSE)、脂连蛋白(APN)、氨基末端脑钠肽前体(NT-proBNP)及炎症因子的影响。方法将2017年6月至2019年5月在本院就诊的80例急性心力衰竭合并快速心律失常患者按随机数字表法分为观察组和对照组。对照组仅给予西医常规治疗。观察组在西医常规治疗的基础上,采用丹红注射液治疗。运用超声心动图评估患者左心室射血分数(LVEF)、室间隔厚度(IVST)、舒张早期最大血流峰值速度/舒张晚期最大血流峰值速度(E/A)的水平。检测2组患者治疗前后血清白细胞介素(IL)-17、IL-35,同型半胱氨酸(Hcy)、NT-proBNP、APN、NSE的水平。对比2组的中医疗效和心电图疗效。结果 2组治疗后LVEF、E/A明显高于治疗前,IVST明显低于治疗前(P<0.05);治疗后,观察组的LVEF、E/A高于对照组,IVST低于对照组(P<0.05)。2组治疗后的IL-17、Hcy明显低于治疗前,IL-35明显高于治疗前(P<0.05);观察组治疗后的IL-17、Hcy低于对照组,IL-35高于对照组,差异有统计学意义(P<0.05)。2组治疗后的NT-proBNP、APN、NSE明显降低(P<0.05);观察组治疗后的NT-proBNP、APN、NSE低于对照组(P<0.05)。观察组的中医疗效高于对照组,差异有统计学意义(P<0.05)。2组心电图疗效差异无统计学意义(P>0.05)。结论丹红注射液能进一步降低急性心力衰竭合并快速心律失常患者的炎症因子的水平,有助于减轻心肌细胞损伤,改善心功能,提高临床疗效。 Objective To investigate the effects of Danhong injection on serum neuron specific enolase(NSE),adiponectin(APN),N-terminal pro-brain natriuretic peptide(NT-proBNP)and inflammatory factors in patients with acute heart failure and tachyarrhythmia.Methods Eighty patients with acute heart failure combined with tachyarrhythmia were divided into observation group and control group.The observation group was treated with Danhong injection on the basis of routine western medicine treatment.The control group was treated with routine western medicine only.The left ventricular ejection fraction(LVEF),interventricular septal thickness(IVST),peak velocity of early diastole and peak velocity of late diastole(E/A)were assessed by echocardiography.Serum levels of interleukin-17(IL-17),IL-35,homocysteine(Hcy),NT-proBNP,APN and NSE were measured before and after treatment.The curative effects of traditional Chinese medicine and electrocardiogram were compared between the two groups.Results After treatment,LVEF and E/A in both groups were significantly higher than those before treatment,and IVST was significantly lower than those before treatment(P<0.05).After treatment,the LVEF and E/A of the observation group were higher than those of the control group,and IVST was lower than that of the control group(P<0.05).After treatment,the levels of IL-17 and Hcy in the two groups were significantly lower than those before treatment,and the levels of IL-35 were significantly higher than those before treatment(P<0.05).After treatment,IL-17 and Hcy in the observation group were lower than those in the control group,and IL-35 was higher than that in the control group.The difference was significant(P<0.05).After treatment,the levels of NT-proBNP,APN and NSE in both groups were significantly decreased(P<0.05).After treatment,the levels of NT-proBNP,APN and NSE in the observation group were lower than those in the control group(P<0.05).The effect of traditional Chinese medicine in the observation group was higher than that in the control group.The difference was significant(P<0.05).There was no significant difference in ECG efficacy between the two groups(P>0.05).Conclusion Danhong injection can further reduce the level of in-flammatory factors in patients with acute heart fail-ure combined with tachyarrhythmia,help to alleviate myocardial cell damage,improve cardiac function and improve clinical efficacy.
