摘要
目的探析中小剂量丙种球蛋白方案对重症免疫性血小板减少性紫癜(ITP)患儿血小板影响的临床意义。方法选取广西壮族自治区南溪山医院2018年3月—2019年2月收治的55例均给予中小剂量丙种球蛋白治疗的重症ITP患儿作为治疗组,观察和比较治疗前后血小板计数(PLT)、平均血小板容积(MPV)、血小板压积(PCT)及血小板分布宽度(PDW)的变化,并与同期来该院接受常规体检的55例正常儿童(对照组)进行比较。结果治疗组患儿治疗前的PLT、MPV、PCT和PDW指标与正常对照组儿童相比,均存在明显统计学差异(P<0.05)。治疗组患儿治疗后的PLT、PCT水平均较治疗前明显提高,而MPV和PDW水平较治疗前明显降低,差异均具有显著性(P<0.05);经治疗后上述指标与正常对照组接近,差异无显著性(P>0.05)。结论中小剂量丙种球蛋白治疗方案并不会对重症免疫性血小板减少性紫癜患儿血小板产生较为严重的不良影响,该治疗方案为此类患儿的治疗提供了新的选择。
Objective To explore the clinical significance of medium or low-dose gamma globulin therapy in children with severe immune thrombocytopenic purpura(ITP).Methods 55 children with severe ITP treated with medium or low-dose gamma globulin from March 2018 to February 2019 in Nanxishan Hospital of Guangxi Zhuang Autonomous Region were selected as the study group,the changes of PLT,MPV,PCT and PDW of platelets before and after treatment were observed and compared with those of 55 normal children(control group)who received routine physical examination during the same period.Results PLT,MPV,PCT and PDW in the study group before treatment were significantly different from those in the control group(P<0.05).The levels of PLT and PCT in the study group were higher than those before treatment,while the levels of MPV and PDW were lower than those before treatment,the difference was statistically significant(P<0.05).Although the above indexes of study group after treatment were close to those in the normal control group,there was no statistical difference between two groups(P>0.05).Conclusions Medium or low dose of gamma globulin does not have a serious adverse effect in children with severe immune thrombocytopenic purpura.It can provide an alternative choice for the clinical treatment of such children.
