摘要
目的观察不同剂量富马酸比索洛尔对经皮冠状动脉介入治疗(PCI)合并抑郁状态的患者心率变异性的影响。方法选取2015年12月至2016年12月我院心内科收治的160例PCI术后合并抑郁状态的患者,按入院顺序单双号分为治疗组及对照组各80例,对照组应用富马酸比索洛尔2.5 mg/d,治疗组根据心率变化,在基础剂量上逐渐增加剂量至10 mg/d,均治疗3个月。两组治疗前后均行24 h动态心电图测定心率变异性,统计恶性心律失常情况,测定抑郁自评量表(SDS)评分,并观察其两组治疗前后抑郁状态及心率变异性指标。结果治疗后,两组SDNN、SDANN、RMSSD、PNN50均高于治疗前,且治疗组高于对照组;治疗后,治疗组SDS评分明显降低,且低于对照组;治疗组恶性心律失常例数明显低于对照组,差异均有统计学意义(P<0.05)。两组不良反应比较差异无统计学意义(P>0.05)。结论PCI术后合并抑郁状态的患者,使用大剂量富马酸比索洛尔可以更好的改善患者抑郁状态,显著增加患者心率变异性,降低室颤阈值,改善其预后。
Objective To investigate the effects of different doses of bisoprolol on PCI patients with depression and heart rate variability.Methods One hundred and sixty patients with depression after PCI were randomly divided into treatment or control group,80 in each group.Patients in the two groups were given bisoprolol treatment.However,the control group was given 2.5 mg/d basic dose for 3 months without change,and in the treatment group,the dosage was gradually increased to 10 mg/d,and maintained for 3 months of treatment.24 h-namic ECG was tested before and after treatment to check heart rate variability and calculate malignant arrhythmia.SDS score and curative effect was measured and depression and heart rate variability before and after treatment were observed.Results After treatment,scores of SDNN,SDANN,RMSSD,and PNN50 were significantly increased in both groups and the treatment group was significantly higher than the control group.After treatment,SDS in the treatment group was significantly decreased and was significantly lower than the control group(P<0.05).The number of malignant arrhythmia cases in the treatment group was significantly less than that in the control group(P<0.05).Conclusion In PCI patients complicated with depression,large dose of bisoprolol could improve their depression,increase the heart rate variability in patients with ventricular fibrillation,and reduce the threshold,thus,improve the prognosis significantly.
