摘要
目的探究经颈动脉多普勒超声(carotid doppler ultrasound,CDU)联合经颅彩色多普勒超声(transcramial color code sonography,TCCS)评估颈内动脉-椎基底动脉吻合(persistent internal carotid⁃basilar anastomosis,PCVBA)血流动力学特点的价值。方法收集PCVBA患者10例,均完成CT血管成像(CTA)、CDU、TCCS检查,记录双侧椎动脉、基底动脉、大脑后动脉血流动力学参数,评估CDU联合TCCS评估PCV⁃BA血流动力学改变的可行性。结果10例患者中永存三叉动脉6例(Ⅰ型3例,Ⅱ型2例,Ⅲ型1例;左侧3例,右侧3例);永存舌下动脉3例(左侧1例,右侧2例);永存寰前节间动脉1例(Ⅰ型,右侧);4例存在后循环梗死及颈内动脉狭窄。结论CDU与TCCS联合评估血流动力学改变初筛永存动脉存在一定的价值,永存三叉动脉、永存舌下动脉及永存寰前节间动脉均有明显血流动力学改变特点,但永存舌下动脉与永存寰前节间动脉超声鉴别困难,需结合CTA进一步验证。
Objective To explore the value of carotid Doppler ultrasound(CDU)combined with transcranial color code sonography(TCCS)in evaluating hemodynamic features of persistent internal carotid⁃basilar anastomosis(PCVBA).Methods Ten patients with PCVBA were selected and examined with CT angiography(CTA),CDU and TCCS.Hemodynamic parameters of bilateral vertebral artery,basilar artery and posterior cerebral artery were recorded.The feasibility of CDU combined with TCCS for evaluating hemodynamic changes of PCVBA was evaluated.Results Among the 10 patients,there were 6 cases with permanent trigeminal artery(3 cases with typeⅠ,2 cases with typeⅡand 1 case with typeⅢ,3 cases on the left and 3 cases on the right),3 cases with permanent sublingual artery(1 case on the left and 2 cases on the right),1 case with permanent proatlantal intersegmental artery(type I,on the right)and 4 cases with posterior circulation infarction and internal carotid artery stenosis.Conclusion CDU combined with TCCS is of certain value in changing permanent arteries through hemodynamics.There are significant hemodynamics changes in permanent trigeminal artery,permanent sublingual artery and permanent proatlantal intersegmental artery.However,ultrasound identification is difficult between permanent sublingual artery and permanent proatlantal intersegmental artery.CTA needs to be combined with to further verify.
