摘要
目的观察"护肝医用冷敷贴"联合西药治疗慢性乙型肝炎患者的临床疗效。方法选择慢性乙型肝炎患者760例,随机分为治疗组和对照组各380例。对照组给予常规抗病毒(恩替卡韦、替诺福韦)、护肝降酶(阿拓莫兰、复方甘草酸苷)治疗,治疗组在对照组治疗基础上加用穴位敷贴,两组均以2周为1个疗程。检测两组患者治疗前后血液分析,包括白细胞计数(WBC)、红细胞计数(RBC)、血小板计数(PLT);以及肝功能指标,包括谷丙转氨酶(ALT)、总胆红素(TBiL)、白蛋白(ALB)、凝血功能(PT);IGG试验(ICGR15)、肝脏实时弹性成像肝弹指数检测(LF index)、脾脏彩超检查(门脉内径、脾脏厚度),并计算治疗前后的肝功能Child-Pugh分级评分,统计患者乏力、纳差、腹胀、厌油等主要症状的变化。结果治疗组治疗后WBC、ALT、TBi L、ALB、PT、LF index、ICG-R15、Child-Pugh评分明显优于对照组,差异有统计学意义(P<0.05);治疗组在改善患者主要症状(乏力、纳差、腹胀、厌油、大便异常)方面明显优于对照组,差异有显著统计学意义(P<0.01)。结论 "护肝医用冷敷贴"联合西药治疗慢性乙型肝炎疗效优于单纯西药组,值得临床应用及推广。
Objective To observe the clinical effect of liver-protecting medical cold compress combined with western medicine on chronic hepatitis B patients. Methods 760 cases of patients with chronic hepatitis B were randomly divided into treatment group and control group,with 380 cases each. Control group was given routine antiviral(entecavir,tenofovir)and hepatoprotective enzymes(atromolan,compound glycyrrhizin);treatment group was given acupoint application on the basis of control group. Both groups were treated for 2 weeks as a course of treatment. Blood analysis(WBC,RBC,PLT),liver function(ALT,TBi L,ALB),coagulation function(PT),IGG test(ICG-R15),real-time elastography of liver(LF index),color Doppler ultrasonography of spleen(portal vein diameter,spleen thickness)were measured before and after treatment. Child-Pugh grading scores of liver function before and after treatment were calculated.The changes of main symptoms such as fatigue,anorexia,abdominal distension and anorexia were counted. Results After treatment,the liver function(ALT,TBi L,ALB),coagulation function(PT),LF index, ICG-R15 and Child-Pugh scores in treatment group were significantly better than those in controlgroup(P<0.05),and the main symptoms(fatigue,anorexia,abdominal distention and anorexia) in treatment group were significantly better than those in control group(P<0.01),indicators(ALT,ICG-R15,TBil)were significantly improved after treatmen(tP<0.01). Conclusion The curative effect of liver-protecting medical cold compress combined with western medicine on chronic hepatitis B is better than that of simple western medicine,which is worthy of clinical application and promotion.
