摘要
目的观察稳斑汤与体外反搏联合治疗冠心病经皮冠状动脉介入(PCI)术后的临床疗效,以及对病人血清血管细胞黏附分子-1(VCAM-1)和单核细胞趋化蛋白-1(MCP-1)的影响。方法选取2014年10月—2016年2月辽宁中医药大学附属医院收治的冠心病PCI术后痰瘀互结证病人60例,采用随机数字表法分为治疗组和对照组,每组30例。对照组采用常规西药联合中药汤剂稳斑汤治疗;治疗组在对照组治疗基础上联合体外反搏治疗。两组均治疗4周。观察两组治疗前后心绞痛症状积分、中医证候积分,采用ELISA双抗体夹心法检测血清VCAM-1和MCP-1水平。结果治疗后,两组心绞痛症状积分、中医证候积分、血清VCAM-1、MCP-1水平均较治疗前明显降低(P<0.05或P<0.01),且治疗组心绞痛症状积分、中医证候积分、血清VCAM-1、MCP-1均较对照组明显降低(P<0.01)。结论稳斑汤联合体外反搏治疗方案能够有效改善冠心病PCI术后病人的临床症状及中医证候,降低血清VCAM-1和MCP-1水平,从而抑制炎症反应。
Objective To observe the clinical effects of Wenban Decoction combined with external counterpulsation on serum vascular cell adhesion molecule-1(VCAM-1)and monocyte chemoattractant protein-1(MCP-1)in coronary heart disease after percutaneous coronary intervention(PCI).Methods Sixty cases of coronary heart disease patients with phlegm-stasis syndrome after PCI were randomly divided into control group and treatment group,with 30 cases in each group.The patients in control group were given conventional western medicine plus Wenban Decoction treatment,the patients in treatment group were given external counterpulsation therapy on the basis of control group,both groups treated for 4 weeks.The score of angina pectoris symptom,the curative effect of angina pectoris symptom,the score of TCM syndrome,and the curative effect of TCM syndrome were observed,the content of serum VCAM-1 and MCP-1 were detected by double antibody sandwich ELISA method.Results Compared with data before treatment,the score of angina pectoris symptom,the score of TCM syndrome,the content of serum VCAM-1and MCP-1 decreased significantly after treatment in two groups(P<0.05 or P<0.01).The score of TCM syndrome,the score of angina pectoris symptom,the content of serum VCAM-1 and MCP-1 in treatment group were lower than those in control group(P<0.01).Conclusion Wenban Decoction combined with external counterpulsation therapy can effectively improve the clinical symptom and TCM syndrome of patients with coronary heart disease after PCI,reduce the content of serum VCAM-1 and MCP-1,and inhibit inflammatory reaction.
