摘要
目的探讨腰椎管狭窄症治疗中后路减压固定联合椎间植骨融合的疗效。方法方便选择腰椎管狭窄症患者80例,均于2017年7月-2019年6月接受治疗,实验组、常规组各40例。实验组接受后路减压固定联合椎间植骨融合治疗,常规组接受单纯椎板减压治疗,观察两组疗效。结果实验组优良率87.5%,显著高于常规组的60.0%(χ~2=7.560,P=0.000)。实验组术中出血量(345.4±31.3)mL、手术时间(122.5±21.0)min、住院时间(11.4±3.3)d,均显著低于常规组的(393.3±37.2)mL、(147.1±26.4)min、(17.3±4.7)d(t=6.340、6.670、8.670,P=0.000)。实验组社会功能(82.4±5.3)分、角色功能(81.5±3.7)分、躯体功能(82.8±4.4)分、认知功能(83.8±4.5)分,均显著高于常规组的(75.3±4.2)分、(71.1±3.4)分、(72.1±3.6)分、(73.2±3.3)分(t=6.230、7.320、7.170、6.980,P=0.000)。结论用后路减压固定联合椎间植骨融合治疗腰椎管狭窄效果显著,可明显减轻患者疼痛,术后患者生活质量更高。
Objective To investigate the efficacy of lumbar spinal stenosis in the treatment of middle and posterior intervertebral fusion and internal fixation. Methods 80 cases of patients with lumbar spinal stenosis were conveniently selected. All patients received treatment in the hospital from July 2017 to June 2019, and were randomly divided into experimental group(n=40) and conventional group(n=40). The conventional group received simple lamina decompression treatment, and the experimental group received posterior decompression and fixation combined with intervertebral bone graft fusion treatment, to observe the efficacy of the two groups. Results The excellent and good rate of the experimental group was 87.5%, which was significantly higher than that of the conventional group by 60.0%(χ~2=7.560, P =0.000). The intraoperative blood loss(345.4±31.3)mL, operation time(122.5±21.0)min and hospital stay(11.4±3.3)d in the experimental group were significantly lower than those in the conventional group(393.3 ±37.2)mL,(147.1±26.4)min and(17.3±4.7)d(t=6.340, 6.670, 8.670, P =0.000). The scores of social function(82.4 ±5.3)points, role function(81.5 ±3.7)points, physical function(82.8±4.4)points and cognitive function(83.8±4.5)points in the experimental group were significantly higher than those in the conventional group(75.3 ±4.2)points,(71.1 ±3.4)points,(72.1 ±3.6)points and(73.2 ±3.3)points(t =6.230, 7.320,7.170, 6.980, P =0.000). Conclusion The treatment of lumbar spinal stenosis with posterior decompression and fixation combined with intervertebral bone graft fusion has a significant effect, which can significantly relieve the pain of patients,and the postoperative quality of life of patients is higher.
