摘要
仝小林教授临床发现女性患者甲状腺、乳腺、子宫三处凡有一处出现结节、增生、肿块,另外两处多有伴随症状产生,甲状腺结节、乳腺增生、子宫肌瘤三种疾病经常相继或者联合发病,因此首先提出"女性三联征"的概念:"女性三联征"是女性甲状腺结节、乳腺增生、子宫肌瘤的统称。三种疾病在致病因素、发病机制等方面有密不可分的联系。现代医学认为甲状腺、乳腺、子宫皆受下丘脑和垂体的调节,三者发病都有内分泌紊乱因素,临床治疗功能上皆从调节内分泌激素入手,当疾病累及器官,造成实质性结构的变化时再加以手术治疗。中医认为气郁为"三联征"共同的主要致病因素,七情不节为其主要病机,痰凝、气滞、血瘀为三者共同的病理产物。三者病因病机、发病规律皆相似,因此三种疾病可做异病同治。仝小林教授根据"三联征"临床症状以虚实不同分为郁而结,虚而结,提出"郁者散之,虚者补之"的治疗大法,治疗多以开郁化痰,散结通络为原则,独辟蹊径提出运用靶方靶药辨病论治,治疗疾病,指导临床实践。
Female triad is the general name of female thyroid nodule,hyperplasia of mammary gland and hysteromyoma. Professor TONG Xiaolin first proposed this concept because he found that clinically women always suffer from these three diseases at the same time or in succession. Thyroid nodule,hyperplasia of mammary glands and uterine leiomyoma often occur successively or jointly. Therefore,the concept of "female triad"is first put forward,and it is believed that the three diseases are inextricably linked in terms of pathogenic factors and pathogenesis. Western medicine holds that the thyroid,breast and uterus are regulated by the hypothalamus and pituitary gland. Diseases of the three parts are due to endocrine disorders. Therefore,the treatment is to start with the regulation of endocrine hormones and surgical treatment afterward. Chinese medicine believes that the etiology,pathogenesis and pathogenesis of the three diseases are similar,so the three diseases can be treated in the same way. Seven emotions disorder is the main cause of illness. Qi stagnation is the main pathogenic factor,while phlegm coagulation,Qi stagnation and blood stasis are the common pathological products. According to the clinical symptoms of"triad sign",Professor TONG Xiaolin divided the diseases into stagnation and deficiency according to the difference of deficiency and excess. He puts forward the treatment method of "relieving stagnation and tonifying the deficiency". The treatment is based on the principle of removing phlegm,dispersing and clearing collaterals. It is a unique way to apply target medicine to treat diseases and guide clinical practice.
