摘要
目的探讨支气管肺泡灌洗液结核杆菌检测联合T-SPOT.TB试验诊断菌阴肺结核的价值,以期提高痰涂片及培养阴性的肺结核患者的结核分枝杆菌检出率,为菌阴肺结核的诊断提供依据。方法选取医院2018年12月-2019年12月收治的菌阴肺结核患者60例作为研究组。另选取非结核性疾病患者60例非结核受试者作对照组。所有研究对象均进行BALF结核杆菌检测及T-SPOT.TB试验。结果研究组T-SPOT阳性率明显高于对照组(P <0.01)。T-SPOT.TB试验及支气管肺泡灌洗液结核杆菌检测单独使用及联合检测时诊断敏感度存在明显差异(P <0.01)。结论 T-SPOT.TB试验及支气管肺泡灌洗液结核杆菌检测联合使用可明显提高菌阴肺结核的诊断率。
Objective To explore the value of detection of Mycobacterium tuberculosis in bronchoalveolar lavage fluid(BALF) combined with T-SPOT.TB test in the diagnosis of sputum negative pulmonary tuberculosis, in order to improve the detection rate of Mycobacterium tuberculosis in patients with sputum smear and culture negative, and to provide basis for the diagnosis of sputum negative pulmonary tuberculosis. Methods 60 patients with bacterial negative pulmonary tuberculosis treated in our hospital from December 2018 to December 2019 were selected as the study group. Another 60 patients with non-tuberculosis diseases were selected as the control group. All subjects were tested for Mycobacterium tuberculosis by BALF and T-SPOT.TB test. Results The positive rate of T-SPOT in the study group was significantly higher than that in the control group(P < 0.01). There was a significant difference in the diagnostic sensitivity between T-SPOT.TB and BALF alone and in combination(P < 0.01). Conclusion The combined use of T-SPOT.TB and detection of Mycobacterium tuberculosis in BALF can significantly improve the diagnostic rate of bacteria-negative pulmonary tuberculosis.
