临床检验基于患者数据室内质量控制方法的研究与进展 被引量:17

摘要 室内质量控制是实验室工作的重要组成部分,传统的室内质量控制通过测定质控品来完成。另一种室内质量控制方法是利用实验室的患者数据,来对实验室误差进行监测,与利用质控品进行质控相比,基于患者数据的质量控制方法具有成本低、无基质效应、无互换性问题、可连续对分析性能进行监测等优点。早在20世纪60年代,基于患者数据的质控方法就已经被提出,但是由于当时计算机技术的限制,这些方法并未得到很好的应用,仅有少数相关的研究对其性能进行评估。21世纪随着信息技术的发展,这些质量控制方法的实现变得比以前容易许多,因此它们也越来越受到实验室人员的关注。该文介绍了一些目前比较常见的基于患者数据的室内质量控制方法,包括差值检查法、正态均值法、Bull算法、指数加权移动均值法、移动中位数法、离群值移动和与移动标准差法等,这些方法可以作为常规室内质控的补充,提高实验室的整体分析质量。 Internal quality control is an important component of clinical laboratories and conventional internal quality control is accomplished through control materials.Making use of patient data from clinical laboratories to monitor laboratory errors is another way of internal quality control.Compared with conventional internal quality control,the advantages of internal quality control methods based on patient data lie in low cost,no matrix effects,no commutability issues and continuous monitoring of analytical performance.As early as 1960s,patient-based quality controls have been proposed.But these quality control methods were not well applied due to the limitations of computer technology at that time and only a few studies evaluated their performance.With the development of information technology in the 21st century,the implementation of these quality control methods has become much easier than before,so they are getting more and more attention from clinical laboratories.This review describes some common internal quality control methods based on patient data,which can be used as supplements to conventional internal quality control to improve overall analytical quality of clinical laboratories,including delta checks,average of normals,Bull algorithm,exponentially weighted moving average,moving median,moving sum of outliers and moving standard deviation.
