摘要
目的探讨超声乳化白内障吸除联合小梁切除术与联合房角分离术治疗原发性急性闭角型青光眼合并白内障的临床效果和安全性。方法83例原发性急性闭角型青光眼合并白内障患者,根据手术方法不同分为对照组(41例)和实验组(42例)。对照组患者采取超声乳化白内障吸除联合小梁切除术进行治疗,实验组患者采取超声乳化白内障吸除联合房角分离术进行治疗。比较两组患者术后视力水平及并发症发生情况。结果实验组患者术后视力水平优于对照组,差异具有统计学意义(χ^2=7.179,P<0.05)。实验组患者并发症发生率为4.76%,低于对照组的19.51%,差异具有统计学意义(χ^2=4.260,P<0.05)。结论原发性急性闭角型青光眼合并白内障患者行超声乳化白内障吸除联合房角分离术,效果优于超声乳化白内障吸除联合小梁切除术,可有效的提高患者视力水平,降低并发症发生率,安全性更高,值得在临床上推广应用。
Objective To discuss the clinical effect and safety of phacoemulsification combined with trabeculectomy and phacoemulsification combined with goniosynechialysis for primary acute angle-closure glaucoma complicated with cataract.Methods A total of 83 cases of primary acute angle-closure glaucoma complicated with cataract were divided into control group(41 cases)and experimental group(42 cases)by different surgical methods.The control group was treated by phacoemulsification combined with trabeculectomy,and the experimental group was treated by phacoemulsification combined with goniosynechialysis.The postoperative visual acuity and occurrence of complications were compared between the two groups.Results The postoperative visual acuity of the experimental group was better than that of the control group,and the difference was statistically significant(χ^2=7.179,P<0.05).The incidence of complications of the experimental group was 4.76%,which was lower than that of the control group 19.51%,and the difference was statistically significant(χ^2=4.260,P<0.05).Conclusion Phacoemulsification combined with goniosynechialysis for primary acute angle-closure glaucoma combined with cataract is better than phacoemulsification combined with trabeculectomy,and it can effectively improve visual acuity,lower the incidence of complications with higher safety,which is worthy of clinical promotion and application.
