摘要
目的 探讨血清胱抑素C(CysC)与尿白介素-18(IL-18)联合检测对脓毒症患者急性肾损伤的诊断价值.方法 回顾性分析2012年6月至2019年6月我院重症医学科就诊的90例脓毒症患者的临床资料,根据是否并发急性肾损伤将患者分为肾损伤组(28例)与无肾损伤组(62例).比较两组血清CysC、尿IL-18水平,分析单一检测与联合检测的诊断价值.结果 肾损伤组的血清CysC、尿IL-18水平均高于无肾损伤组(P<0.05).血清CysC联合尿IL-18检测对脓毒症并发急性肾损伤的诊断灵敏度、准确率均高于单一血清CysC、尿IL-18检测(P<0.05).结论 联合检测血清CysC、尿IL-18对脓毒症患者急性肾损伤具有良好的早期预测和诊断价值.
Objective To explore the diagnostic value of combined detection of serum cystatin C(CysC)and urine interleukin-18(IL-18)in sepsis patients with acute renal injury.Methods The clinical data of 90 patients with sepsis visited in the critical medicine department of our hospital from June 2012 to June 2019 were analyzed retrospectly and the patients were divided into renal injury group(28 cases)and non-renal injury group(62 cases)according to whether they had acute renal injury.The levels of serum CysC and urine IL-18 were compared between the two groups,and the diagnostic value of single detection and combined detection were analyzed.Results The levels of serum CysC and urine IL-18 in the renal injury group were significantly higher than those in the non-renal injury group(P<0.05).The sensitivity and accuracy of serum CysC combined with urine IL-18 in the diagnostic of sepsis complicated with acute renal injury were higher than those of single serum CysC or urine IL-18(P<0.05).Conclusion The combined detection of serum CysC and urine IL-18 has good early prediction and diagnostic value for acute renal injury in sepsis patients.
