摘要
目的探讨三七合枕清眠安汤联合通督益脑安神针治疗卒中后失眠气滞血瘀证患者的临床疗效。方法选择北京中医医院顺义医院2017年6月—2019年3月收治的216例卒中后失眠气滞血瘀证患者作为研究对象,按照随机数字表法分为对照组和观察组各108例。对照组患者给予艾司唑仑片口服,观察组患者在口服艾司唑仑片基础上加三七合枕清眠安汤联合通督益脑安神针治疗,2组均治疗1个月。观察2组患者治疗前后匹兹堡睡眠质量指数(PSQI)评分、脑卒中专门化生活质量量表(SS-QOL)积分、中医症状评分、症状自评量表(SCL-90)积分及血清5-羟色胺(5-HT)、多巴胺(DA)水平变化情况,比较2组患者的治疗效果。结果与治疗前相比,2组患者治疗后PSQI中各单项评分和总分、中医症状积分、SCL-90积分及血清DA水平均明显降低(P均<0.05),SS-QOL积分及血清5-HT水平均明显升高(P均<0.05);与对照组比较,观察组PSQI中各单项评分和总分、中医症状积分、SCL-90积分及血清DA水平降低均更为显著(P均<0.05),SS-QOL积分及血清5-HT水平升高均更为显著(P均<0.05)。观察组治疗有效率为95.37%(103/108),对照组为74.07%(80/108),观察组显著高于对照组(P<0.05)。结论三七合枕清眠安汤联合通督益脑安神针治疗卒中后失眠气滞血瘀证,可以明显改善患者临床症状,提高睡眠和生活质量,可能与调节5-HT、DA分泌有关。
Objective It is to explore the clinical effect of Sanqi plus Zhenqingmian’an decoction combined with Tongdu Yinao Anshen acupuncture on insomnia of syndrome of Qi stagnation and blood stasis after stroke.Methods A total of 216 patients with post-stroke insomniaof syndrome of Qi stagnation and blood stasis who were admitted to Shunyi Hospital of Beijing Hospital of Traditional Chinese Medicine from June 2017 to March 2019 were selected as the research objects.They were divided into control group and observation group according to random number table method,108 cases in each group.The patients in the control group were given estazolam tablets orally,and the patients in the observation group were treated with estazolam tablets and Sanqi plus Zhenqingmian’an decoction combined with Tongdu Yinao Anshen acupuncture.Both groups were treated for 1 month.The changes of Pittsburgh Sleep Quality Index(PSQI)scores,SS-QOL scores,TCM symptom scores,SCL-90 scores and the levels of serum 5-hydroxytryptamine(5-HT)and dopamine(DA)before and after treatment were observed in the two groups,and the treatment effect of the two groups were compared.Results Compared with before treatment,the individual scores and total scores of PSQI,TCM symptom scores,SCL-90 scores and serum DA level of the two groups were significantly reduced after treatment(all P<0.05),SS-QOL scores and the serum 5-HT level was significantly increased(P<0.05).Compared with the control group,the decease of individual scores and total scores of PSQI,TCM symptom score,SCL-90 score and serum DA level and increase of SS-QOL score and serum 5-HT level in the observation group were more significantly than those in the control group(P<0.05).The effective rate of treatment in the observation group was 95.37%(103/108),and that in the control group was 74.07%(80/108),the observation group was significantly higher than the control group(P<0.05).Conclusion Sanqi plus Zhenqingmian'an decoction combined with Tongdu Yinao Anshen acupuncture can significantly improve the clinical symptoms,improve sleep and quality of life in the treatment of insomnia of syndrome of Qi stagnation and blood stasis after stroke,the mechanism may be related to the regulation of 5-HT and DA secretion.
