摘要
[目的]构建急性心肌梗死病人介入术后分层随访路径。[方法]采用全球急性冠状动脉事件注册评分(GRACE)进行风险分层,初步构建不同病情危险程度的急性心肌梗死病人出院后的随访路径。通过两轮德尔菲法专家函询,确定最终的分层随访路径。[结果]两轮函询问卷的有效回收率均为100%,专家权威系数为0.85,两轮肯德尔协调系数分别为0.11(P<0.05)、0.21(P<0.05);最终确定的分层随访路径包括5个一级条目,中低危病人随访路径包括26个二级条目,高危病人在此基础上调整补充为16个二级条目。[结论]急性心肌梗死病人介入术后分层随访路径明确了不同病情风险等级的急性心肌梗死病人的随访时间、形式和内容,为开展院外延续性分层随访管理提供参考。
Objective:To construct a hierarchical follow-up path for patients with acute myocardial infarction after intervention surgery.Methods:The global registry of acute coronary events scores(GRACE)was used to stratify the risk.And the follow⁃up paths of patients with acute myocardial infarction(AMI)with different risk degrees were preliminarily constructed.Through two rounds of Delphi expert consultations,the final hierarchical follow⁃up path was determined.Results:The effective recovery rate of the two rounds of questionnaires were both 100%.Authority coefficient of expert was 0.85.And the Kendall coordination coefficient of two rounds were 0.11(P<0.05)and 0.21(P<0.05),respectively.The final hierarchical follow⁃up path consisted of 5 first⁃level entries.Follow⁃up path of patients at low and medium risk included 26 second⁃level entries.In addition,follow⁃up path of patients at high risk increased 16 second⁃level entries.Conclusion:The hierarchical follow⁃up path of patients with acute myocardial infarction after intervention surgery clarifies the follow⁃up times,forms and contents of patients with acute myocardial infarction at different disease risk levels,which provides reference for the continuing management of hierarchical follow⁃up outside hospital.
