慢性肾脏病3~5期患者肾功能与皮质醇水平的相关性分析 被引量:4

Correlation analysis of renal function and cortisol levels in patients with chronic kidney disease 3 to 5 stage

摘要 目的探讨慢性肾脏病(CKD)3~5期患者肾功能与皮质醇(COR)水平的相关性。方法选取2018年1月至2019年6月该院收治的CKD3~5期患者96例为CKD组,其中CKD3期组20例,CKD4期组16例,CKD5期组60例;选取同期该院体检健康者34例为对照组。比较CKD组与对照组COR、促肾上腺皮质激素(ACTH)、尿素(Urea)、血肌酐(Scr)、乳酸脱氢酶(LDH)、总蛋白(TP)、清蛋白(Alb)、总胆固醇(TC)、三酰甘油(TG)、预估肾小球滤过率(eGFR)水平;比较CKD3~5期组与对照组的上述各指标水平;采用Pearson相关分析COR水平与eGFR水平的相关性。结果CKD组COR、ACTH、Scr、Urea、LDH水平均高于对照组(P<0.05),eGFR、Alb水平均低于对照组(P<0.05);TP、TC、TG水平在两组间比较,差异均无统计学意义(P>0.05)。CKD3~5期组COR、ACTH、Scr、Urea水平均高于对照组(P<0.05),eGFR水平均低于对照组(P<0.05);随着肾功能下降,COR、Scr、Urea水平逐渐升高(P<0.05)。COR水平与eGFR水平呈负相关(r=-0.591,P<0.001)。结论CKD3~5期患者存在COR水平的变化,且随着肾功能下降,COR水平逐渐升高。 Objective To investigate the correlation between renal function and cortisol(COR)levels in patients with chronic kidney disease(CKD)3 to 5 stage.Methods From January 2018 to June 2019,96 patients with CKD 3 to 5 stage treated in the hospital were selected as CKD group,including 20 patients in CKD 3 stage group,16 patients in CKD 4 stage group and 60 patients in CKD 5 stage group.Thirty-four healthy people accepted health examination in the same period were selected as the control group.The levels of COR,adrenocorticotropic hormone(ACTH),Urea,serum creatinine(Scr),lactate dehydrogenase(LDH),total protein(TP),albumin(Alb),total cholesterol(TC),triacylglycerol(TG),estimated glomerular filtration rate(eGFR)were compared between the CKD group and control group;the levels of the above indicators were compared between the CKD 3 to 5 stage group and control group;Pearson correlation was used to analyze the correlation between COR level and eGFR level.Results The levels of COR,ACTH,Scr,Urea and LDH in the CKD group were higher than those in the control group(P<0.05),and the levels of eGFR and Alb in the CKD group were lower than those in the control group(P<0.05);there was no significant difference in TP,TC and TG levels between the two groups(P>0.05).The levels of COR,ACTH,Scr and Urea in the CKD 3 to 5 stage group were higher than those in the control group(P<0.05),and the level of eGFR in the CKD 3 to 5 stage group was lower than that in the control group(P<0.05).With the decreased of renal function,the levels of COR,Scr and Urea increased gradually(P<0.05).There was a negative correlation between COR level and eGFR level(r=-0.591,P<0.001).Conclusion COR level in patients with CKD 3 to 5 stage is changed,and with the decrease of renal function,COR level gradually increase.
