摘要
目的通过与胸水中结核菌核酸检测法比较,评价IL-27在检测结核性胸膜炎中的诊断价值。方法选择2017年11月至2018年5月无锡市第五人民医院结核科收治的53例结核性胸膜炎患者和49例恶性胸腔积液患者,检测两组患者胸水及血清中IL-27水平,同时对两组患者胸水进行结核菌核酸检测,对检测结果进行统计分析。结果结核组患者胸水IL-27浓度为(516±14)ng/L,高于恶性组(313±87)ng/L(P<0.001),恶性组患者血清IL-27浓度虽高于结核组,但差异无统计学意义。胸水IL-27应用ROC曲线诊断结核性胸膜炎的敏感度为88.0%,特异度为78.0%。胸水IL-27检测诊断结核性胸膜炎的敏感度高于结核菌核酸检测(P<0.001)。结论胸水IL-27检测用于诊断结核性胸膜炎具有较高的敏感度和特异度,是辅助诊断结核性胸膜炎的一种较为简便、高效的方法。
Objective To evaluate the diagnostic value of IL-27 in the detection of tuberculous pleurisy by comparing with the detection of tuberculous bacterial nucleic acid in pleural effusion.Methods From November 2017 to May 2018,53 patients with tuberculous pleurisy and 49 patients with malignant pleural effusion were selected from the Tuberculosis Department of Wuxi No´5 People´s Hospital.The levels of IL-27 in pleural effusion and serum of the two groups were detected,and tuberculosis nucleic acid in pleural effusion was detected,then the results were analyzed statistically.Results The concentration of IL-27 in pleural effusion of tuberculosis group was(516±14)ng/L,which was higher than that of malignant group(313±87)ng/L(P<0.001).The concentration of IL-27 in malignant group was higher than that in tuberculosis group,but there was no significant difference.The sensitivity and specificity of ROC curve in the diagnosis of tuberculous pleurisy were 88.0%and 78.0%respectively.The sensitivity of IL-27 detection in pleural effusion in the diagnosis of tuberculous pleurisy was higher than that of tuberculous bacterium nucleic acid(P<0.001).Conclusion The detection of IL-27 in pleural effusion has high sensitivity and specificity in the diagnosis of tuberculous pleurisy,and it is a simple and efficient method for assistant diagnosis.
