摘要
目的探讨基于人文关怀理念的优质护理服务在妊娠期糖尿病高龄产妇剖宫产围手术期护理中的应用效果。方法选取2018年1月-2019年6月于本院行剖宫产手术分娩的80例高龄并妊娠期糖尿病产妇作为研究对象,采用随机数表法将其分为护理组和对照组两组,每组各40例。对照组产妇接受常规剖宫产围手术期护理管理,护理组则接受基于人文关怀的优质护理服务。对比两组患者术前血糖控制情况及分娩结局。结果两组产妇入院时空腹血糖、餐后2 h血糖水平比较无显著差异(t=1.049、1.196,P>0.05),产前1 d时血糖均较入院时明显降低,但护理组产前1 d时空腹血糖、餐后2 h血糖水平均明显低于对照组(t=4.566、9.613,P<0.05);两组术后7 d、14 d时VAS评分均明显低于术后次日(t1=2.936、4.613,t2=3.031、4.795,P<0.05),且术后次日两组VAS评分比较无显著差异(t=1.244,P>0.05),但护理组术后7 d、14 d时VAS评分明显低于对照组(t=3.032、2.849,P<0.05);两组产妇产前1 d时SAS、SDS评分比较无显著差异(t=1.250、1.037,P>0.05),产后7 d是护理组SAS、SDS评分均明显低于对照组(t=7.416、9.526,P<0.05);护理组产妇产后并发症发生率明显低于对照组,并且护理组产妇护理满意度明显高于对照组(χ2=12.387、8.278,P<0.05)。结论对合并妊娠期糖尿病的高龄产妇剖宫产围手术期实施基于人文关怀的优质护理服务可有效改善产妇血糖水平,并对抑制产妇围产期焦虑、抑郁等不良情绪以及降低产后不良反应、并发症发生风险具有积极作用,具有临床应用及推广价值。
Objective To explore the application effect of high quality nursing service based on humanistic care concept in perioperative nursing care of senior parturients with gestational diabetes mellitus undergoing cesarean section.Methods 80 seniorpregnant women with gestational diabetes mellitus who underwent cesarean section in our hospital from January 2018 to June 2019 were selected as the study subjects.They were divided into nursing group and control group according to random number table method,and 40 cases in each group.40 cases in control group received perioperative nursing management of routine cesarean section,and 40 cases in nursing group received quality nursing service based on humanistic care.Preoperative blood sugar control and delivery outcomes were compared between the two groups.Results There was no significant difference in fasting blood glucose and 2 h postprandial blood glucose between the two groups when they were admitted to the hospital(t=1.049,1.196,P>0.05).The levels ofblood sugar were decreased both two groups at one day before they giving birth,however,the fasting blood glucose and 2 h postprandial blood glucose in the nursing group were significantly lower than those in the control group(t=4.566,9.613,P<0.05).VAS scores at 7 thand 14 thday after operation were significantly lower than those at the next day after cesarean section(t1=2.936,4.613,t2=3.031,4.795,P<0.05).There was no significant difference in VAS score between the two groups on the next day after operation(t=1.244,P>0.05),but the VAS score of the nursing group was significantly lower than that of the control group on the 7 th and 14 th day after operation(t=3.032,2.849,P<0.05);there was no significant difference in SAS and SDS score between the two groups on the day before delivery(t=1.250,1.037,P>0.05),and the scores of SAS and SDS in the nursing group on the 7 th day after delivery were significantly lower than those in the control group(t=7.416,9.526,P<0.05).The incidence of complications was significantly lower in the nursing group than in the control group,and the nursing satisfaction of the puerpera in the nursing group was significantly higher than that in the control group(χ2=12.387,8.278,P<0.05).Conclusions The implementation of high-quality nursing service based on humanistic care during the perioperative period of caesarean section for the senior parturients with gestational diabetes mellitus could effectively improve the blood sugar level of parturients,and it also has positive effect on inhibiting the bad mood such as perinatal anxiety,depression and reducing the risk of postpartum adverse reactions and complications,and has clinical application and popularization value.
