摘要
目的观察补肺活血胶囊联合布地奈德混悬液雾化吸入治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并肺部感染患者的疗效及对免疫功能和血气指标的影响。方法选取2018年6月-2019年3月河南中医药大学第二附属医院收治的AECOPD患者94例,按随机数字表随机分为对照组和研究组,各47例。对照组给予布地奈德混悬液雾化吸入,研究组在对照组基础上加用补肺活血胶囊。比较两组患者治疗的有效率、免疫功能指标[T淋巴细胞(CD3^+、CD4^+、CD8^+)、自然杀伤细胞(Natural killer cell,NK)],血气分析指标[动脉血氧分压(Partial pressure of oxygen 2,PaO2)、动脉血二氧化碳分压(Partial pressure of carbon dioxide,PaCO2)],肺功能指标[一秒用力呼气容积(Forced expiratory volume in one second,FEV1)、用力呼气量占用力肺活量比值(FEV1/FVC)、最大呼气流量(Peak expiratory flow,PEF)]及内皮功能指标[内皮素-1(Endothelin-1,ET-1)、一氧化氮(Nitric oxide,NO)]水平。结果研究组患者的治疗总有效率93.62%高于对照组的78.72%,两组比较差异有统计学意义(P<0.05);研究组患者的CD3^+、CD4^+、NK细胞水平均高于对照组,CD8^+水平低于对照组(P<0.05);研究组患者的PaO_2高于对照组、PaCO2低于对照组(P<0.05);研究组患者FEV1、FEV1/FVC、PEF水平均高于对照组(P<0.05);研究组患者ET-1低于对照组、NO高于对照组(P<0.05)。结论补肺活血胶囊联合布地奈德混悬液雾化吸入可显著降低AECOPD患者的炎症因子水平,提高动脉血氧分压,控制肺部感染,缓解其临床症状。
OBJECTIVE To observe the effect of Bufeihuoxue capsules combined with budesonide suspension aerosol inhalation on treatment of the acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients complicated with pulmonary infection and analyze the impact on the immune function and blood gas indexes.METHODS A total of 94 patients with AECOPD who were treated in Second Affiliated Hospital of Henan University of Traditional Chinese Medicine from Jun 2018 to Mar 2019 were enrolled in the study and randomly divided into the control group and the study object,with 47 cases in each group.The control group was treated with budesonide suspension aerosol inhalation,while the study group was given additional Bufeihuoxue capsules on basis of the treatment of the control group.The effective rate of treatment,immune function indexes[T lymphocytes(CD3^+,CD4^+,CD8^+),natural killer cell,(NK)],blood gas indexes[partial pressure of oxygen 2,(PaO2),partial pressure of carbon dioxide(PaCO2)],pulmonary function indexes[forced expiratory volume in one second(FEV1),ratio of forced expiratory volume to forced vital capacity(FEV1/FVC),peak expiratory flow(PEF)]and endothelial function indexes[endothelin-1(ET-1),nitric oxide(NO)]were observed and compared between the two groups of patients.RESULTS The total effective rate of the study group was 93.62%,higher than 78.72%of the control group,and there was significant difference between the two groups(P<0.05).The levels of CD3^+,CD4^+and NK cells of the study group were significantly higher than those of the control group,the CD8^+level of the study group was significantly significantly lower than that of the control group(P<0.05).The PaO2 of the study group was significantly higher than that of the control group,the PaCO2 of the study group was significantly lower than that of the control group(P<0.05).The levels of FEV1,FEV1/FVC and PEF of the study group were significantly higher than those of the control group(P<0.05).The ET-1 of the study group was lower than that of the control group,while the NO of the study group was higher than that of the control group(P<0.05).CONCLUSION Bufeihuoxue capsules combined with budesonide suspension aerosol inhalation can significantly reduce the levels of inflammatory factors of the patients with AECOPD,raise the arterial oxygen partial pressure,control lung infection and relieve clinical symptoms.
