摘要
目的:系统评价清热活血类中药对自身免疫性前列腺炎(EAP)模型相关炎症因子干预作用的有效性。方法:检索中国知网、中国生物医学文献数据库、维普中文科技期刊数据等,筛选清热活血类中药对EAP模型干预实验研究,制作文献资料筛选路径并实行资料提取,采用SYRCLE动物实验偏倚风险评估工具对纳入文献研究进行偏倚风险评估,并用Rev Man 5.3软件对结局指标进行Meta分析。结果:共纳入10个研究,研究对象为Wistar大鼠或昆明(KM)小鼠,均为随机对照实验。Meta分析结果显示:与模型组相比,清热活血类中药可降低炎性因子IL-1水平(SMD=-2.52,95%CI[-3.27,-1.77])、IL-6水平(SMD=-1.54,95%CI[-2.30,-0.78])、IL-8(SMD=-1.45,95%CI[-1.94,-0.97)、IL-17水平(SMD=-3.52,95%CI[-4.61,-2.44]),抑制TNF-α表达(SMD=-3.78,95%CI[-5.53,-2.02]),P<0.000 01;对IFN-γ水平无明显影响(SMD=-0.47,95%CI[-1.09,0.15],P=0.14)。异质性分析显示,P<0.000 01,I2=92%,显示两组异质性较大。结论:清热活血类中药能够有效抑制EAP模型动物前列腺内炎症因子IL-1、IL-6、IL-8、IL-17、TNF-α的表达,存在一定的量效关系,但对IFN-γ无明显影响。
Objective:To systematically evaluate the efficacy of clearing heat and activating blood medicinals on experimental autoimmune prostatitis(EAP).Methods:CNKI,CBMdisc,VIP,Wanfang Data,CBMdisc,PubMed,Embase and Cochrane Library were searched and experimental study of EAP treated with clearing heat and activating blood medicinals were retrieved.Data extraction tables were formulated and the data were extracted.Risk assessment of the methodological bias of the included literature was performed by using the SYRCLE Animal Experiment Bias Risk Assessment Tool and the meta-analysis of outcome indicators was conducted with RevMan 5.3 software.Results:A total of 10 studies were included in the study.The subjects were Wistar rats or KM mice,and all the studies were randomized controlled trials.Meta-analysis showed that compared with model group,clearing heat and activating blood medicinals could decrease the level of inflammatory factors such as interleukin(IL)-1(SMD=-2.52,95%CI[-3.27,-1.77],P<0.00001),IL-6(SMD=-1.54,95%CI[-2.30,-0.78],P<0.0001),IL-8(SMD=-1.45,95%CI[-1.94,-0.97],P<0.00001),IL-17(SMD=-3.52,95%CI[-4.61,-2.44],P<0.00001),and had no obvious effect on interferon(IFN)-γ(SMD=-3.78,95%CI[-5.53,-2.02],P<0.0001).It also could decrease the level of tumor necrosis factor(TNF)-α(SMD=-3.78,95%CI[-5.53,-2.02],P<0.0001),heterogeneity analysis(P<0.00001,I2=92%)showed that the data of two groups were with high heterogeneity.Conclusions:Clearing heat and activating blood medicinals can effectively decrease the level of IL-1,IL-6,IL-8,IL-17 and TNF-αin a dose-dependent manner,but had no obvious effect on IFN-γ.
