摘要
目的探讨冠心病发病的影响因素及其与骨相关蛋白(骨保护素和骨钙素)的关系。方法选择60例冠心病患者(冠心病组)、30例志愿者(对照组)。测定所有研究对象的空腹血糖、糖化血红蛋白、LDL-C、血清肌酐、ALP、空腹胰岛素水平,以及血清骨保护素、骨钙素、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)水平,并计算胰岛素敏感指数(ISI)、男性者比例、高血压和2型糖尿病患病比例。比较两组一般资料以及上述各指标的差异,并分析冠心病发病的影响因素及其与骨相关蛋白的关系。结果与对照组比较,冠心病组的年龄、男性者比例、高血压比例、2型糖尿病比例、空腹血糖、糖化血红蛋白、FINS、血肌酐、骨保护素、骨钙素、TNF-α、IL-1β水平均升高,ISI水平降低(均P<0.05)。多因素Logistic回归分析结果显示,男性、空腹胰岛素、骨保护素、IL-1β水平升高是冠心病发生的危险因素(均P<0.05)。多元线性回归分析结果显示,患2型糖尿病、IL-1β水平与冠心病患者骨保护素水平呈正相关(均P<0.05)。结论男性、空腹胰岛素、骨保护素、IL-1β水平升高均为冠心病发病的影响因素。患2型糖尿病及IL-1β水平与骨保护素水平呈正相关,两者可能影响骨保护素对冠心病的作用。
Objective To explore the influencing factors of incidence of coronary heart disease and its relationship with bone-related proteins(osteoprotegerin and osteocalcin). Methods Sixty patients with coronary heart disease (coronary heart disease group) and 30 volunteers (control group) were selected. The levels of FBG, hemoglobin A1c(HbA1c), LDL-C, serum creatinine, ALP and fasting insulin(FINS), as well as the levels of serum osteoprotegerin, osteocalcin, tumor necrosis factor α(TNF-α) and interleukin 1β(IL-1β) were determined in all research subjects, and insulin sensitivity index(ISI), male ratio, and the proportions of hypertension and type 2 diabetes cases were calculated. The differences in clinical data as well as the indices mentioned above were compared between the two groups, in addition, the influencing factors of incidence of coronary heart disease and its relationship with bone-related proteins were analyzed. Results Compared with the control group, the coronary heart disease group had increased levels of age, male ratio, proportions of hypertension and type 2 diabetes, FBG, HbA1c, FINS, serum creatinine, osteoprotegerin, osteocalcin, TNF-α and IL-1β, and decreased ISI level (all P <0.05). Multivariate Logistic regression analysis results showed that males and elevated levels of FINS, osteoprotegerin and IL-1β were the risk factors for developing coronary heart disease (all P <0.05). Multiple linear regression analysis results showed that suffering from type 2 diabetes or IL-1β level positively correlated with osteoprotegerin level in patients with coronary heart disease (all P <0.05). Conclusion Males and the levels of FINS, osteoprotegerin and IL-1β are the influencing factors of developing coronary heart disease. Suffering from type 2 diabetes and IL-1β level positively correlate with osteoprotegerin level, and both may affect the effect of osteoprotegerin on coronary heart disease.
