摘要
目的探究多囊卵巢综合征(PCOS)患者的抗苗勒管激素(AMH)、抑制素B(INHB)和稳态模型胰岛素抵抗指数(HOMA-IR)水平检测及其诊断价值。方法将温州市中医院于2017年8月-2019年1月收治的97例PCOS患者作为PCOS组,根据睾酮(T)值水平的不同将其分为高T值组(55例)和正常T值组(42例)。另选择同一时期在本院进行健康体检的50例志愿者作为对照组。观察比较PCOS组、对照组的AMH、INHB和HOMA-IR水平,并对PCOS组中高T值组、正常T值组的AMH、INHB和HOMA-IR水平进行对比。采用受试工作特征曲线(ROC曲线)分析AMH、INHB、HOMA-IR以及三项联合检测对PCOS的临床诊断价值,计算其曲线下面积(AUC)、敏感度和特异度。结果PCOS组的AMH、INHB和HOMA-IR水平均明显高于对照组(P<0.05)。PCOS组中高T值组的AMH、INHB及HOMA-IR水平均明显高于正常T值组(P<0.05)。三项联合检测的AUC为0.852,高于AMH、INHB、HOMA-IR单一检测。三项联合检测在诊断PCOS的敏感度和特异度分别为94.55%、92.86,均高于AMH、INHB、HOMA-IR。结论PCOS患者会出现AMH、抑制素B及HOMA-IR水平显著升高。AMH、INHB、HOMA-IR三者联合在诊断PCOS的敏感度和特异度均高于单项检测,是诊断PCOS的重要参考指标。
Objective To research the detection and diagnostic value of anti-Mullerian hormone(AMH),inhibin B(INHB)and homeostasis model insulin resistance index(HOMA-IR)in patients with polycystic ovary syndrome.Methods From August 2017 to January 2019,97 patients with PCOS were divided into two groups:high T group(55 cases)and normal T group(42 cases)according to the level of testosterone(T).Another 50 volunteers who underwent physical examination in our hospital during the same period were selected as the control group.The levels of AMH,INHB and HOMA-IR in PCOS group and control group were observed and compared,and the levels of AMH,INHB and HOMA-IR in high T value group and normal T value group in PCOS group were compared.The clinical diagnostic value of AMH,INHB,HOMA-IR and three combined tests for PCOS was analyzed by ROC curve.The area under curve(AUC),sensitivity and specificity were calculated.Results The levels of AMH,INHB and HOMA-IR in PCOS group were significantly higher than those in control group(P<0.05).The levels of AMH,INHB and HOMA-IR in PCOS group with high T value were significantly higher than those in normal T value group(P<0.05).The AUC of three combined tests was 0.852,higher than that of AMH,INHB and HOMA-IR(0.819,0.566 and 0.618,respectively).The sensitivity and specificity of the three combined tests in the diagnosis of PCOS were 94.55%and 92.86%respectively,which were higher than those of AMH,INHB and HOMA-IR(81.82%,52.73%,60.00%and 73.81%,64.29%and 66.67%,respectively).Conclusion The levels of AMH,inhibin B and HOMA-IR in PCOS patients increased significantly.The sensitivity and specificity of the combination of AMH,INHB and HOMA-IR in the diagnosis of PCOS are higher than those of single detection,which is an important reference index for predicting and diagnosing PCOS.
