摘要
目的探讨入院评估在急性肾损伤患者持续肾脏替代治疗中的应用及对患者肾功能的影响。方法选择2018年4月—2019年4月的急性肾损伤患者作为研究对象,根据时间点的不同分为对照组(2018年4月—10月)和观察组(2018年11月—2019年4月),每组各40例患者。对照组给予常规护理干预,观察组在对照组基础上联合入院评估。10 d后对患者的效果进行评价,比较两组血气水平、肾功能水平及护理依从性。结果观察组干预后10 d PaO2、PaCO2均高于对照组(P<0.05);两组治疗后10 d血肌酐、尿素氮水平均低于治疗前(P<0.05),且观察组低于对照组(P<0.05);观察组干预后10 d护理方法、CRRT治疗、遵医用药、饮食干预、康复锻炼依从性均高于对照组(P<0.05)。结论入院评估用于急性肾损伤患者持续性肾脏替代治疗中能改善患者血气水平及肾功能,提高护理依从性,值得推广应用。
Objective To investigate the application of admission assessment in continuous renal replacement therapy for patients with acute kidney injury and its effect on renal function.Methods 80 patients with acute kidney injury from April 2018 to April 2019 were selected as subjects,and were divided into control group(April 2018—October 2018)and observation group(November 2018—April 2019),40 patients in each group.The control group was given routine nursing intervention.The observation group was evaluated based on the admission of the control group.After 10 days,the patient′s effect was evaluated.The blood gas level,renal function level,and nursing compliance were compared between the two groups.Results The levels of PaO2,PaCO2,10 days after the intervention in the observation group were all higher than those in the control group(P<0.05);the blood creatinine and urea nitrogen levels in the two groups were lower than those before treatment(P<0.05);The levels of serum creatinine and urea nitrogen at 10 days after treatment were lower than those in the control group(P<0.05);the observation group′s compliance with nursing methods,CRRT treatment,compliance with medical drugs,diet intervention,and rehabilitation exercise were higher than those in the control group at 10 days after the intervention(P<0.05).Conclusion Admission evaluation for continuous renal replacement therapy in patients with acute kidney injury can improve patients′blood gas levels and renal function,improve nursing compliance,and is worth promoting and applying.
