摘要
目的:评估Q开关激光治疗咖啡斑的疗效及相关影响因素。方法:将2010年1月-2018年1月笔者科室门诊接受Q开关激光治疗的资料完整的66例咖啡斑患者随机分为QS-532组(32例)和QS-755组(34例)。QS-532组采用Q开关532nm激光治疗,QS-755组采用Q开关755nm激光治疗。每3个月治疗1次,对比治疗后6个月、9个月和12个月时的临床疗效。QS-755组再根据患者年龄、性别、部位、咖啡斑颜色、形状、边缘情况、面积进行分组,比较治疗前后的临床变化,观察激光治疗的不良反应。结果:随访至初次治疗后12个月,QS-755组的有效率和痊愈率分别为73.5%和41.2%,高于QS-532组的46.9%和28.1%(P<0.05)。根据咖啡斑的颜色分析,浅褐色(χ2=12.628,P<0.05)及中褐色(χ2=11.972,P<0.05)的咖啡斑临床疗效均优于深褐色咖啡斑,而浅褐色与中褐色咖啡斑的临床疗效差异无统计学意义(χ2=0.922,P>0.05);根据咖啡斑形状的情况分析,形状不规则的有效率明显高于形状规则的咖啡斑(χ2=12.871,P<0.05);根据咖啡斑边缘的情况,边缘不光滑的咖啡斑有效率明显高于边缘光滑的咖啡斑(χ2=10.557,P<0.05);不同面积的咖啡斑治疗效果差异无统计学意义(χ2=0.172,P>0.05);不同性别患者治疗效果的差异无统计学意义(χ2=0.156,P>0.05);不同年龄阶段接受治疗的临床疗效差异无统计学意义(χ2=0.189,P>0.05);不同部位咖啡斑治疗的临床疗效差异无统计学意义(χ2=0.137,P>0.05)。治疗过程中除疼痛外未发生其他不良反应。结论:Q开关755nm激光是治疗咖啡斑的一种安全有效的方法,其疗效受咖啡斑颜色、形状及边缘情况的影响。
Objective To evaluate the curative effect and the influencing factors of Q-switch laser treatment in Café-au-laitmacules(CALMs).Methods 66 patients of CALMs accepted Q-switch laser treatment,adopted by dermatology clinic in Xinjiang Uygur Autonomous Region People’s Hospital,were divided in 2 groups,QS-532(32 cases)accepted Q-switch laser treatment of 532nm and QS-755(34 cases)accepted Q-switch laser treatment of 755nm.Both of them were accepted by the Q-switch laser treatment every three months.The clinical efficacy was compared at 6,9 and 12 months after treatment.Group QS-755 were further divided into groups according to age,gender,location,color,shape,edge and area.The clinical changes before and after treatment were compared.The adverse reactions of laser therapy were observed.Results Following-up for 12 months after the first treatment,the effective rate and cure rate of QS-755 were 73.5%and 41.2%,higher than QS-532 of 46.9%and 28.1%(P<0.05).According to color analysis of CALMs,the clinical effects of sandy beige(χ2=12.628,P<0.05)and toffee(χ2=11.972,P<0.05)were superior to puce CALMs,while,there was no statistical significance(χ2=0.922,P>0.05)between sandy beige and toffee in the clinical effects.According to form analysis,the clinical effects of irregular ones were apparently better than regular ones(χ2=12.871,P<0.05).According to border analysis,the clinical effects of unsmooth ones had advantages over smooth ones(χ2=10.557,P<0.05).However,There was no significant difference in the therapeutic effect between the different gender(χ2=0.156,P>0.05).There was no significant difference in the therapeutic effect between the different area(χ2=0.172,P>0.05).There was no statistical significance in the clinical effects in different ages(χ2=0.189,P>0.05).There was no significant difference in the therapeutic effect between the different location(χ2=0.137,P>0.05).Besides,there was no side effect other than a little pain in the treatment.Conclusion Q-switch laser treatment of 755 nm is a safe and effective method to treat CALMs,depending on the color,border and form.
