摘要
目的探讨外周血造血干细胞(peripheral blood stem cell,PBSC)采集对健康供者血钾水平的影响,并进行原因分析,制订相应的护理对策。方法2017年3月至2018年8月,便利抽样法选择在某医院血液内科行PBSC采集的133例健康供者为研究对象,连续收集其PBSC采集期间的血钾指标,观察并记录相应的临床症状、血钾水平,分析低血钾症的发生原因。结果133例供者PBSC采集术前血钾水平均正常[(3.9±0.3)mmol/L],采集后血清钾水平降低[(3.2±0.4)mmol/L],差异有统计学意义(t=12.321,P<0.01);术后有110例供者发生低钾血症,发生率为82.71%;不同程度的低钾血症供者临床症状的发生率差异无统计学意义(均P>0.05)。结论应密切监测健康供者在PBSC采集过程中的血钾水平及相关临床症状,尽早发现低钾血症的发生并及时制订相应的医护策略予以纠正,以最大程度地减轻供者的不适,保障供者安全。
Objective To explore the effect of mobilization and collection of peripheral blood stem cell(PBSC)on blood potassium level of healthy donors,so as to analyze the causes and develop nursing countermeasures.Methods A total of 133 healthy donors underwent collection of PBSC in the hospital from March 2017 to August 2018 were included.The blood potassium indexes were collected continuously during the period of PBSC collection,the corresponding clinical symptoms and blood potassium levels were observed and recorded,and the causes of hypokalemia were analyzed.Results Blood potassium level of 133 patients with PBSC was normal before operation[(3.9±0.3)mmol/L],serum potassium level decreased after operation[(3.2±0.4)mmol/L],the difference was statistically significant(t=12.321,P<0.01);hypokalemia occurred in 110 patients after operation,the incidence was 82.71%;there was no significant difference in the incidence of clinical symptoms among donors with different degrees of hypokalemia(all P>0.05).Conclusions More attention should be paid to the blood potassium level and related clinical symptoms of healthy donors during PBSC collection,find out the occurrence of hypokalemia as early as possible and make corresponding medical strategies to correct it in time,so as to minimize the discomfort and ensure the safety of the donors.
