摘要
目的讨论老年缺血性脑卒中病人的睡眠质量、抑郁发生情况及相关危险因素。方法选择2017年1月至2019年3月在我院接受诊断与治疗的老年缺血性脑卒中病人222例,分析病人抑郁发生情况并将其分为抑郁组与非抑郁组;借助匹兹堡睡眠质量指数量表(PSQI)评估2组病人睡眠状况,利用简明健康测量量表(SF-36)测定2组病人生存质量;收集研究对象一般情况并分析老年缺血性脑卒中病人发生抑郁的相关危险因素。结果222例老年缺血性脑卒中病人中,抑郁发生率为41.44%(92/222);抑郁组病人PSQI量表得分明显高于非抑郁组,SF-36量表各维度得分均明显低于非抑郁组,差异均具有统计学意义(P<0.05);Logistic多因素分析结果显示,女性、卒中前性格内向、卒中位置在左侧、脑力劳动者、无配偶、个人收入<1000元、高中以上文化程度、中重度神经功能受损为老年缺血性脑卒中病人抑郁的独立危险因素(P<0.05)。结论老年缺血性脑卒中病人抑郁发生率较高,且病人普遍存在睡眠质量低下、生存质量低下的情况,应给予具备抑郁高危因素病人更多的关注,在最大程度上预防老年缺血性脑卒中后抑郁的发生。
Objective To discuss the sleep quality,the incidence of depression and related risk factors in the elderly patients with ischemic stroke.Methods A total of 222 elderly patients with ischemic stroke who were diagnosed and treated in our hospital from January 2017 to March 2019 were selected.The patients were divided into depression group and non-depression group;The incidence of depression was analyzed.Pittsburgh sleep quality index(PSQI)was used to evaluate the sleeping status of the two groups.The MOS item short form healthy survey(SF-36)was used to measure the quality of life of the two groups.The related risk factors of depression in the elderly patients with ischemic stroke were analyzed.Results The incidence rate of depression was 41.44%(92/222).The score of PSQI in depression group was significantly higher than that in non-depression group,and the score of SF-36 was significantly lower than that in non-depression group(P<0.05).Multivariate Logistic analysis revealed that women,introverted personality before stroke,stroke location on the left,mental workers,no spouse,personal income<1000 RMB,high education level,moderate to severe neurological impairment were independent risk factors for depression in the elderly patients with ischemic stroke(P<0.05).Conclusions The incidence of depression in the elderly patients with ischemic stroke is high,and the patients generally have poor sleep quality and poor quality of life.More attention should be paid to the patients with high risk factors of depression,so as to prevent the occurrence of depression in the elderly patients with ischemic stroke to the greatest extent.
