肾功能正常的冠心病患者血清FGF-23与冠状动脉钙化的相关性 被引量:3

Correlation between serum FGF-23 and coronary artery calcification in asymptomatic coronary heart disease patients with preserved renal function

摘要 目的探讨肾功能正常的无症状冠心病患者血清成纤维细胞生长因子23(fibroblast growth factor 23, FGF-23)水平与冠状动脉钙化之间的联系。方法收集行CT冠状动脉成像(computed tomography coronary angiography, CTCA)的肾功能正常患者150例。检测所有患者的血清FGF-23、磷、钙、甲状旁腺激素(PTH)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、三酰甘油、血肌酐、糖化血红蛋白等指标。计算所有患者CTCA结果的冠状动脉钙化积分。结果血清FGF-23水平与血磷呈正相关(r=0.210,P=0.014),与肾小球滤过率呈负相关(r=-0.240,P=0.021);冠状动脉钙化积分与年龄(r=0.386,P<0.001)、糖化血红蛋白(r=0.217,P=0.032)、FGF-23(r=0.214,P=0.024)呈正相关,与肾小球滤过率呈负相关(r=-0.216,P=0.032);多重线性回归分析示,FGF-23、年龄、1,25-(OH)2-D3与冠状动脉钙化积分独立相关(βFGF-23=0.20,P=0.014)。结论在肾功能正常的患者中,血清FGF-23与冠状动脉钙化独立相关,早期检测FGF-23对评估患者CAC水平具有重要指导价值。 Objective To investigate the correlation between serum FGF-23 level and coronary artery calcification in asymptomatic coronary heart disease patients with preserved renal function.Methods From January 2014 to May 2018, 150 patients with preserved renal function who underwent computed tomography coronary angiography(CTCA) were included. The serum levels of FGF-23, LDL-c, triglyceride, serum creatinine and glycosylated hemoglobin were measured in all patients. Coronary artery calcification score was calculated in all patients with CTCA results. Results Serum FGF-23 level was significantly positively correlated with serum phosphorus(r=0.21, P=0.014) and negatively correlated with glomerular filtration rate(r=-0.24, P=0.021). Coronary artery calcification score was significantly positively correlated with age(r=0.386, P<0.001), glycosylated hemoglobin(r=0.217, P<0.032), and FGF-23(r=0.214, P<0.024);and negatively correlated with glomerular filtration rate(r=-0.216, P=0.032). Multiple linear regression analysis showed that FGF-23, age and 1,25-(OH)2-D3 were independently correlated with coronary calcification score. Conclusion In patients with preserved renal function, serum FGF-23 is an independent risk factor of coronary artery calcification. Early detection of FGF-23 has important guiding value for assessing CAC levels in patients.
