摘要
目的探讨斜外侧腰椎椎间融合(OLIF)联合后路内固定治疗退行性腰椎侧凸的临床疗效。方法回顾性分析自2017-03—2018-06采用OLIF联合后路内固定治疗的18例退行性腰椎侧凸,比较术前、术后1周以及末次随访时伤椎Cobb角、腰椎前凸角、疼痛VAS评分、ODI指数。结果 18例均顺利完成手术,随访时间平均9.4(2~17)个月。术后1例出现cage位置靠前,后方椎间隙撑开幅度较小;2例出现患侧大腿前侧麻木,屈髋无力;2例出现术侧大腿皮温升高;1例背部切口出现脂肪液化,对症处理后均治愈,未出现血管及输尿管损伤等严重并发症。术后1周及末次随访时伤椎Cobb角、腰椎前凸角、疼痛VAS评分、ODI指数较术前均明显改善,差异有统计学意义(P <0.05)。结论 OLIF联合后路内固定治疗退行性腰椎侧凸有效地矫正了矢状面和冠状面的平衡,症状改善明显,近期疗效满意。
Objective To investigate the clinical efficacy of oblique lateral lumbar interbody fusion(OLIF) combined with posterior internal fixation for degenerative lumbar scoliosis. Methods A retrospective analysis of 18 cases of degenerative lumbar scoliosis treated with OLIF combined with posterior internal fixation from March 2017 to June 2018 was conducted.The injured Cobb angle, lumbar lordosis angle, Pain VAS score, ODI index before surgery, 1 week after surgery, at the last follow-up were compared. Results All 18 patients successfully completed the operation, with an average follow-up time of9.4(2-17) months. In 1 case, the cage was placed anteriorly, and the posterior intervertebral space was stretched to a lesser extent. In 2 cases, the anterior thigh was numb and hip flexion was weak. In 2 cases, the skin temperature of the thigh was increased. Liquefied, cured after symptomatic treatment. In 1 case, fat liquefaction in the back incision, cured after symptomatic treatment. No serious complications such as vascular and ureteral injuries occurred. The Cobb angle, lumbar lordosis angle, pain VAS score, and ODI index of the injured vertebrae at 1 week and the last follow-up were significantly improved after operation, the difference was statistically significant(P <0.05). Conclusion OLIF combined with posterior internal fixation for degenerative lumbar scoliosis effectively corrects the balance of the sagittal and coronal planes. The symptoms are significantly improved and the short-term effect is satisfactory.
