摘要
目的观察肝硬化门静脉高压伴脾功能亢进的介入治疗效果。方法回顾性分析2016年12月-2018年3月阳新县人民医院收治的肝硬化门静脉高压伴脾功能亢进患者100例,根据治疗方法不同分为观察组和对照组,每组50例。对照组行开腹脾切除术,观察组行脾动脉栓塞术(PSE),比较2组治疗前后血小板(PLT)计数、白细胞(WBC)计数、门静脉压(FPP)水平及肝功能相关指标。结果治疗后,2组PLT、WBC水平均高于治疗前,FPP低于治疗前,且观察组优于对照组(P<0.05或P<0.01);观察组丙氨酸氨基转移酶(ALT)水平低于对照组,白蛋白(ALB)、总胆红素(TBil)水平均高于对照组(P<0.05或P<0.01)。结论PSE治疗肝硬化门静脉高压伴脾功能亢进患者效果较好,值得临床推广应用。
Objective Observe the effect of interventional therapy on portal hypertension with cirrhosis and hypersplenism.Methods A retrospective analysis of 100 patients with portal hypertension and splenic hyperfunction in liver cirrhosis admitted to Yangxin County People’s Hospital from December 2016 to March 2018,According to different treatment methods,they were divided into the observation group and the control group,with 50 cases in each group.The control group underwent open splenectomy,and the observation group underwent splenic artery embolization(PSE),Before and after treatment,the platelet(PLT)count,white blood cell(WBC)count,portal vein pressure(FPP)level and liver function related indexes were compared between the two groups.Results After treatment,the levels of PLT and WBC in the two groups were higher than before treatment,FPP was lower than before treatment,and the observation group was superior to the control group(P<0.05 or P<0.01);The levels of alanine aminotransferase(ALT)in the observation group were lower than those in the control group,and the levels of albumin(ALB)and total bilirubin(TBil)were higher than those in the control group(P<0.05 or P<0.01).Conclusion PSE is effective in the treatment of patients with liver cirrhosis and portal hypertension with hypersplenism,which is worthy of clinical application.
