摘要
目的探讨可吸收螺钉联合缝线固定治疗投弹骨折的临床疗效。方法回顾性分析2014年1月至2019年1月收治的58例投弹骨折患者的临床资料,其中31例采用可吸收螺钉联合缝线固定治疗(缝线组),27例采用钛板内固定治疗(钛板组),比较两组患者切口长度、手术时间、骨折愈合时间、末次随访Mayo肘关节功能评分、桡神经损伤发生率等指标的差异。结果随访时间10~36个月,平均随访时间(18.7±3.6)个月。缝线组、钛板组的切口长度和桡神经损伤发生率分别为(6.1±1.0)、(11.3±1.2)cm和3%、18%,两组比较,差异均有统计学意义(P<0.05),但两组手术时间、骨折愈合时间及末次随访Mayo肘关节功能评分并无统计学差异(P>0.05)。结论可吸收螺钉联合缝线固定治疗投弹骨折安全可行,与钛板内固定相比,其具有操作简单、创伤小、桡神经损伤发生率低、无需二次手术取出内固定物等优点。
Objective To analyze the clinical effects of absorbable screws combined with suture fixation in treatment of throwing fracture of the humeral shaft.Methods Clinical data of 58 patients with throwing fracture of the humeral shaft treated from January 2014 to January 2019 were retrospectively analyzed.The patients were divided into two groups,suture group(n=31)and titanium plate group(n=27).Patients in suture group were treated by absorbable screws combined with suture fixation,while patients in titanium plate group were performed with the plate internal fixation.Differences of incision length,operation time,fracture healing time,Mayo elbow function score at the latest follow-up,the incidence of radial nerve injury were compared between two groups.Results All patients were followed up for 10 to 36 months with an average time of(18.7±3.6)months.The incision length and incidence of radial nerve injury in suture group and titanium plate group were(6.1±1.0),(11.3±1.2)cm and 3%,18%respectively,which showed statistical differences between two groups(P<0.05).Operation time,fracture healing time and Mayo elbow function score at the latest follow-up in two groups were similar(P>0.05).Conclusions Absorbable screws combined with suture fixation is safe and feasible for treatment of throwing fracture of the humeral shaft.Compared with titanium plate internal fixation,it has advantages of simplicity,less injury,lower incidence of radial nerve injury,and no need of reoperation to remove the internal fixators.
