摘要
目的:观察双辛鼻鼽散对阳虚肺寒型过敏性鼻炎(Allergic Rhinitis,AR)患者的干预效果。方法:选取2017年2月-2018年10月在北京中医药大学第三附属医院纳入阳虚肺寒型过敏性鼻炎患者。采用自身前后对照方法,给予双辛鼻鼽散(北京市康仁堂药业有限公司,颗粒剂型),2次/日,1包/次,连续服用2周,观察不同时间点(服药前、服药后第1周、服药后第2周、服药后第3个月)患者鼻部症状视觉模拟量表、症状体征分级记分及中医证候积分的变化情况。结果:(1)共纳入70例阳虚肺寒型过敏性鼻炎患者,不同时间点鼻部症状视觉模拟量表(Visual Analogue Scale,VAS)分值随用药时间延长而逐渐下降,差异有统计学意义(P<0.001);VAS总分值在用药第3个月下降的比例占100%,各项分值以流涕、鼻塞两项下降的最为明显。(2)用药治疗后的临床疗效总有效率逐渐增长,不同时间点的整体临床疗效差异具有统计学意义(P<0.001)。(3)不同时间点的中医次症,整体以鼻涕清稀、精神倦怠、恶风怕冷、喜热饮、乏力、容易感冒等表现所占比例高。精神倦怠、咳痰色白在不同时间点的差异无统计学意义(P>0.05),其余各项在不同时间点的差异均有统计学意义(P<0.05),其中恶风怕冷、口淡不渴、喜热饮在不同时间点的差异比较最显著(P<0.01)。(4)用药1周时的不良反应共有18例,用药2周时共有3例。用药第2周、第3个月不良反应发生率明显低于第1周,但差异无统计学意义(P>0.05)。结论:双辛鼻鼽散有助于改善过敏性鼻炎患者的临床症状,远期疗效优于短期疗效,用药2周即可对过敏性鼻炎患者发挥一定的预防作用;双辛鼻鼽散对过敏性鼻炎患者流涕、鼻塞症状的治疗效果胜过对鼻痒、喷嚏的治疗效果;双辛鼻鼽散对中医次症的疗效以恶风怕冷、口淡不渴、喜热饮的疗效最佳,优于对其他中医次症的疗效;双辛鼻鼽散的临床疗效显著,不良反应发生率低,且均可缓解,没有远期不良反应,值得临床推广与应用。
Objective:To observe the intervention effect of the Shuangxin Biqiu powder(双辛鼻鼽散)on allergic rhinitis of Yangxu Feihan type(阳虚肺寒型).Methods:From February 2017 to October 2018,patients with allergic rhinitis of Yangxu Feihan type were enrolled and given the Shuangxin Biqiu powder for 2 consecutive weeks.Results:70 patients with allergic rhinitis of Yangxu Feihan type were included.The visual analogue scale score of nasal symptoms at different time points were gradually decreased with the prolonged medication time(P<0.001).The proportion of total VAS scores were decreased by 100%in the 3 rd month of medication,and the most significant declines were runny nose and nasal congestion.The clinical efficacy in the first week after treatment was gradually increased to 48.44%,75.41%in the second week,and 93.44%in the third month.(P<0.001).TCM syndromes at different time points have a high proportion of nasal discharge,mental burnout,Efeng Pale,enjoy hot drinks,fatigue,and cold.There was no significant difference in mental burnout and sputum color at different time points(P>0.05),and the differences of the other items at different time points were statistically significant(P<0.05).The difference between thirst and hot drinks at different time points was the most significant(P<0.01).A total of 18 adverse reactions occurred at one week of treatment and 3 cases at two weeks of treatment.The incidence of adverse reactions was lower in the second and third weeks of medication than the first week,but the difference was not statistically significant(P>0.05).Conclusion:The Shuangxin Biqiu powder can improve the clinical symptoms of patients with allergic rhinitis,and its long-term effect is better than short-term effect.It can play a certain preventive effect on allergic rhinitis after 2 weeks of treatment.The Shuangxin Biqiu powder is more effectiveon runny nose and nasal congestion symptoms than nasal itching and sneezing,and has the best effect on TCM secondary symptoms.The Shuangxin Biqiu powder has significant clinical efficacy,low incidence of adverse reactions,and can alleviate,no longterm adverse side effects,worthy of clinical promotion and application.
