低管电压、管电流、对比剂剂量、浓度与流速在非肥胖患者全腹CTA中的应用 被引量:25

Application of Low Tube Voltage,Tube Current,Contrast Agent Dose,Concentration and Flow Rate in Non-Obese Patients with Whole Abdominal CTA

摘要 目的探讨多层螺旋CT低管电压、管电流、低对比剂剂量、浓度与流速在BMI≤24 kg/m2患者中全腹CT血管造影(CT Angiography,CTA)的可行性。方法将87例BMI≤24 kg/m2并行全腹CTA的患者随机分为两组,A组(n=41)采用100 kV、200 mAs、50 mL(320 mgI/mL)造影剂用量、3 mL/s对比剂注射速率进行血管成像,B组(n=46)采用120 kV、250 mAs、90 mL(350 mgI/mL)造影剂用量、4 mL/s对比剂注射速率进行血管成像。比较两组患者的有效剂量、L2水平腹主动脉、腹腔干、肠系膜上动脉、右肾动脉、右髂内动脉层面CT值、信噪比(Signal-to-Noise Ratio,SNR)、对比信噪比(Contrast-to-Noise Ration,CNR)和图像评分。结果两组患者CT容积剂量指数、剂量长度乘积、有效辐射剂量差异均具有统计学意义(P<0.001)。两组间同层面CT值差异均具有统计学意义(P<0.001),背景噪声(t=1.786,P>0.05)、SNR(t=1.356,P>0.05)、CNR(t=1.615,P>0.05)差异皆不具有统计学意义。A、B两组评分差异不具有统计学意义(Z=-0.326,P>0.05)。结论BMI≤24 kg/m2的患者进行低管电压、管电流、对比剂剂量、浓度与流速的全腹CTA可获得满足诊断的图像,虽然未提升图像SNR、CNR,但明显降低了患者所受辐射剂量与造影剂使用量。 Objective To investigate the feasibility of low tube voltage,tube current,contrast agent dose,concentration and flow rate of multi-slice spiral CT in patients with BMI≤24 kg/m2.Methods A total of 87 patients with BMI≤24 kg/m2 and concurrent abdominal CT Angiography(CTA)were randomly divided into two groups.Patients in A Group(n=41)used 100 kV,200 mAs,50 mL(320 mgI/mL)contrast agent dosage,3 mL/s contrast agent injection rate for vascular imaging.In B group(n=46),angiography was performed with 120 kV,250 mAs,90 mL(350 mgI/mL)contrast agent dosage,and 4 mL/s contrast agent injection rate.The effective dose,L2 level of abdominal aorta,celiac trunk,superior mesenteric artery,right renal artery,and right internal iliac artery were compared between the two groups of patients with CT values,signal-to-noise ratio(SNR),contrast-to-noise ration(CNR),and image scores.Results The differences in CT volume dose index,dose length product,and effective radiation dose between the two groups of patients were statistically significant(P<0.001).The difference of CT value in the same level between the two groups was statistically significant(P<0.001).The differences of background noise(t=1.786,P>0.05),SNR(t=1.356,P>0.05),CNR(t=1.615,P>0.05)were not statistically significant.There was no significant difference in scores between A and B groups(Z=-0.326,P>0.05).Conclusion Patients with BMI≤24 kg/m2 can perform diagnosis of whole abdominal CTA with low tube voltage,tube current,contrast agent dose,concentration and flow rate.Although the SNR and CNR of the image are not improved,the radiation dose and the use of contrast agent are significantly reduced.
