摘要
目的探讨自拟培土生金汤联合寒热并用法治疗慢阻肺急性加重期的疗效。方法选取2018年4月-2019年4月医院收治的98例慢阻肺急性加重期患者,依据入院顺序的不同将其分为对照组48例与试验组50例,对照组施以常规西药治疗,低流量吸氧,氧浓度为28%~30%;沙美特罗替卡松粉吸入剂,1吸/次,2次/d;头孢曲松钠他唑巴坦钠2. 0 g/次,2次/d;盐酸氨溴索30 mg/次,3次/d。试验组施以自拟培土生金汤联合寒热并用法治疗:自拟培土生金汤;寒热并用法,采用蠲饮泄肺法治疗进行治疗,观察两组患者的临床治疗效果、随访1年内急性加重发作次数情况、治疗前及治疗2周后的白细胞介素-8(IL-8)、白细胞介素-23(IL-23)与肿瘤坏死因子α(TNF-α)的变化情况。结果治疗前,两组IL-8、IL-23、TNF-α水平无差异(P> 0. 05);治疗2周后,试验组的IL-8、IL-23、TNF-α水平显著降低,并且试验组的临床治疗总有效率明显提升,随访1年内急性加重发作次数明显减少(P <0. 05)。结论自拟培土生金汤联合寒热并用法治疗慢阻肺急性加重期具有显著的临床效果。
Objective To explore the effect of self-made Peitu Shengjindecoction combined with cold and heat method on acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods 98 patients with acute exacerbation of COPD admitted to our hospital from April 2018 to April 2019 were divided into control group (n = 48) and experimental group (n =50) according to the order of admission. The control group was treated with conventional western medicine,low flow oxygen inhalation,oxygen concentration of 28% ~ 30%;salmeterol roticasone powder inhalation,once inhalation,twice a day;cefuroxime 0. 5 g/time,twice a day;ammonia hydrochloride,twice a day;bromosol 30 mg/time,3 times/d. The experimental group was treated with self-made Peitu Shengjindecoction combined with cold and heat method. The clinical therapeutic effect,the number of acute exacerbations,and the changes of IL-8,IL-23 and TNF-α before and after treatment were observed. Results Before treatment,the levels of IL-8,IL-23 and TNF-alpha had no difference between the two groups (P> 0. 05). After 2 weeks of treatment,the levels of IL-8,IL-23 and TNF-alpha in the experimental group decreased significantly,and the total effective rate of clinical treatment in the experimental group increased significantly. The number of acute exacerbations decreased significantly within 1 yearfollow-upcompared with that of the control group (P < 0. 05). Conclusion The self-made Peitu Shengjindecoction combined with cold and heat method has significant clinical effecton acute exacerbation of chronic obstructive pulmonary disease.
