摘要
目的:探讨自拟疏风通窍汤对慢性鼻窦炎鼻息肉患者术后治疗效果及鼻黏膜恢复的影响。方法:选取2017年1月至2019年6月兴化市人民医院收治的慢性鼻窦炎鼻息肉患者105例,按照随机数字表法将患者分为对照组(n=52)和研究组(n=53)。两组患者均给予内镜鼻窦手术治疗,术后均用0.9%氯化钠溶液冲洗;研究组患者在此基础上给予自拟疏风通窍汤治疗,连续治疗4周。观察两组患者的临床疗效,术后1、4和12周黏浓涕、血痂及黏膜水肿肥厚的发生情况,鼻黏膜纤毛传输速率(MTR)、窦腔黏膜上皮化时间和6个月复发情况。结果:研究组患者的临床总有效率为98.11%(52/53),明显高于对照组的82.69%(43/52),差异有统计学意义(P<0.05)。与术前相比,两组患者术后4周、术后12周的MTR明显增大,差异均有统计学意义(P<0.05);且研究组患者术后4周、术后12周的MTR明显大于对照组,差异均有统计学意义(P<0.001)。术后4周,研究组患者黏浓涕、血痂及黏膜水肿肥厚发生率明显低于对照组,差异均有统计学意义(P<0.05);术后12周,研究组患者所有症状均消失,对照组少量患者仍有症状,两组患者黏浓涕、血痂及黏膜水肿肥厚发生率的差异有统计学意义(P<0.05)。研究组患者术后窦腔黏膜上皮化时间明显短于对照组,6个月复发率明显低于对照组,差异均有统计学意义(P<0.05)。结论:慢性鼻窦炎鼻息肉患者内镜鼻窦手术后应用自拟疏风通窍汤治疗的临床疗效显著,能够明显改善患者临床症状,促进鼻黏膜恢复,且复发率低。
OBJECTIVE:To probe into the effects of Shufeng Tongqiao decoction on postoperative treatment of chronic sinusitis and nasal polyps and recovery of nasal mucosa.METHODS:Totally 105 patients with chronic sinusitis and nasal polyps admitted into Xinghua People’s Hospital from Jan.2017 to Jun.2019 were extracted to be divided into the control group(n=52)and the study group(n=53)via the random number table.Both groups were treated with endoscopic sinus surgery and washed with 0.9%normal saline after surgery.On this basis,the study group received Shufeng Tongqiao decoction for 4 weeks.The clinical efficacy,sticky nose,incidence of blood scabs and mucosal edema and hypertrophy after surgery of 1,4 and 12 weeks,nasal mucociliary transport rate(MTR),sinus mucosa epithelialization time and recurrence at 6 months of two groups were observed.RESULTS:The total clinical effective rate in the study group was 98.11%(52/53),significantly higher than that in the control group 82.69%(43/52),the difference was statistically significant(P<0.05).Compared with before surgery,MTR of two groups increased significantly at 4 and 12 weeks after surgery,and the difference was statistically significant(P<0.05);and MTR of the study group was significantly greater than that of the control group at 4 and 12 weeks after surgery,the difference was statistically significant(P<0.001).After surgery of 4 weeks,the incidence of sticky nose,incidence of blood scabs and mucosal edema and hypertrophy of the study group was significantly lower than that of the control group,with statistically significant difference(P<0.05).After surgery of 12 weeks,all symptoms disappeared in the study group,while a small number of patients in the control group still had symptoms.The difference in the incidence of sticky nose,incidence of blood scabs and mucosal edema and hypertrophy was statistically significant between two groups(P<0.05).The postoperative sinus mucosa epithelialization time of the study group was significantly shorter than that of the control group,and the recurrence rate at 6 months was significantly lower than that of the control group,with statistically significant differences(P<0.05).CONCLUSIONS:The clinical efficacy of Shufeng Tongqiao decoction on postoperative treatment of chronic sinusitis and nasal polyps is remarkable,which can improve the clinical symptoms,promote the recovery of nasal mucosa,and the recurrence rate is lower.
