摘要
目的:探讨后腹腔镜手术在小儿肾积水中的应用效果及对肾功能与肾纤维化指标的表达影响.方法:选取2016年1月-2019年6月本院78例肾积水患儿为研究对象,将其根据手术方式的不同分为对照组(开放式手术)与观察组(后腹腔镜手术),各39例.比较两组术中出血量、手术时间、住院时间、手术前后肾实质厚度(RPT)、肾段动脉阻力指数(RI)、肾功能及肾纤维化指标.结果:观察组术中出血量低于对照组,且手术时间及住院时间均短于对照组(P<0.05).术后1、3个月,观察组的RPT均高于对照组,而肾段动脉RI均低于对照组(P<0.05).术后1、3个月,观察组各项肾功能指标均低于对照组(P<0.05).术后1、3个月,观察组各项肾纤维化指标均低于对照组(P<0.05).结论:后腹腔镜手术在小儿肾积水中的应用效果较好,且对肾功能及肾纤维化指标的表达影响更为积极,具有较高的应用价值.
Objective:To investigate the effect of retroperitoneal laparoscopy in pediatric hydronephrosis and its influence on renal function and renal fibrosis index.Method:A total of 78 children with hydronephrosis in our hospital from January 2016 to June 2019 were selected as study subjects.They were divided into control group(open surgery)and observation group(retroperitoneal laparoscopic)according to different surgical methods,with 39 cases in each group.Intraoperative blood loss,operative time,length of hospital stay,renal parenchymal thickness(RPT),renal segmental arterial resistance index(RI),renal function and renal fibrosis before and after surgery were compared between the two groups.Result:The amount of intraoperative blood loss in the observation group was lower than that in the control group,and operative time and length of hospital stay were lower than those in the control group(P<0.05).1 month and 3 months after surgery,the RPT of the observation group were higher than those of the control group,while the renal segmental arterial RI were lower than those of the control group(P<0.05).1 month and 3 months after surgery,all renal function indexes of the observation group were lower than those of the control group(P<0.05).1 month and 3 months after surgery,all indexes of renal fibrosis in the observation group were lower than those in the control group(P<0.05).Conclusion:Retroperitoneal laparoscopic has a good effect in pediatric hydronephrosis,and has a more positive effect on the expression of renal function and renal fibrosis indexes,with a higher application value.
