巨大卵泡膜细胞瘤的MRI诊断及临床分析 被引量:1

摘要 目的:探讨巨大卵泡膜细胞瘤(ovarian thecoma,OT)的磁共振(magnetic resonance imaging,MRI)表现及临床特点,以提高诊疗水平。方法:回顾性分析2016-01~2019-04我院经手术病理证实的9例巨大OT患者的MRI表现及临床资料。结果:9例患者共10个肿瘤,肿瘤平均最大直径为15.4 cm,边界清楚,包膜完整;MRI表现为实性肿瘤3个、囊实性肿瘤6个及囊性肿瘤1个;平扫所有肿瘤实性成分T1加权像(T1 weighted image,T1WI)呈等或稍低信号,T2加权像(T2 weighted image,T2WI)呈较高或高信号;囊性成分均为T1WI低信号、T2WI高信号,增强扫描3个肿瘤实性成分呈不均匀中度强化,另3个呈较明显延迟强化,所有囊性成分未见强化。3例合并子宫内膜增厚,2例见卵巢囊肿,4例伴发子宫肌瘤,6例见盆腔少量积液。结论:MRI可以清晰显示巨大OT的部位、大小、形态、信号特征及强化方式,对其诊断及鉴别诊断具有重要意义,一旦确诊应争取根治性手术切除。 Objective:To investigate the magnetic resonance imaging(MRI)findings and clinical characteristics of giant ovarian thecoma(OT),in order to improve the diagnostic treatment.Methods:MRI and clinical data of 9 cases with giant OT confirmed by operative pathology from January 2016 to April 2019 were analyzed retrospectively.Results:There were 10 tumors of 9 cases,the mean maximum diameter of tumor was 15.4 cm,the boundary were clear and the capsule were intact.MRI showed that three were solid tumors,six were cystic and solid tumors and one was cystic tumour.On unenhanced MRI,the solid components of all tumors showed equal or slightly low signal on T1 weighted image(T1WI),and slightly high or high signal on T2 weighted image(T2WI),but the cystic components were low signal intensity on T1WI and high signal intensity on T2WI.There were three tumors showed inhomogeneous moderate enhancement,and the other three showed obvious delayed enhancement,and all cystic components showed no enhancement.There were 3 cases with endometrial thickening,2 cases with ovarian cyst,4 cases with uterine leiomyoma and 6 cases with small amount of pelvic effusion.Conclusion:MRI can clearly display the location,morphology,signal characteristics and enhancement mode of giant OT,which has great significance for its diagnosis and differential diagnosis,and radical resection should be removed once diagnosed.
