摘要
目的:探究颈动脉彩超与经颅多普勒(TCD)联合检查在脑卒中筛查中的应用价值.方法:选取本院2018年1-12月诊治的脑卒中患者74例为观察组,选取同期因头疼、头晕就诊的非脑卒中患者70例为对照组.以数字减影血管造影(DSA)作为金标准,两组均行颈动脉彩超与TCD检查,观察颈动脉斑块检测情况、联合检测诊断效能.结果:观察组IMT增厚率、斑块检出率(91.89%、75.68%)高于对照组(31.43%、32.86%),差异均有统计学意义(P<0.05);观察组斑块溃疡发生率37.50%高于对照组4.35%,差异均有统计学意义(P<0.05);颈动脉彩超、TCD联合检查脑卒中敏感度、准确度均高于单项检查,差异均有统计学意义(P<0.05).结论:颈动脉彩超联合TCD检查可早期发现血管病变,明确病变程度及部位,对脑卒中早期预防、治疗与预后具有重要临床价值.
Objective:To explore the value of carotid color Doppler ultrasound and transcranial Doppler(TCD)examination in stroke screening.Method:A total of 74 cases with stroke in our hospital from January to December 2018 were selected as the observation group,70 cases of non-stroke patients who had a headache and dizziness during the same period were selected as the control group,the digital subtraction angiography(DSA)as the gold standard,carotid color Doppler ultrasound and TCD were performed in both groups to observe carotid plaque detection and the combined detection and diagnosis efficacy.Result:The IMT thickening rate and plaque detection rate in the observation group were 91.89%and 75.68%higher than those in the control group of 31.43%and 32.86%,the differences were statistically significant(P<0.05).The incidence of plaque ulcers in the observation group was 37.50%higher than that in the control group(4.35%),the difference was statistically significant(P<0.05).The sensitivity and accuracy of combined carotid color Doppler ultrasound and TCD for stroke detection were higher than those of the individual tests,the difference was statistically significant(P<0.05).Conclusion:Carotid color Doppler ultrasound combined with TCD can detect vascular lesions at an early stage,clarify the extent and location of the lesions,and have important clinical value for the early prevention,treatment and prognosis of stroke.
